Impact of Healthcare Worker Workload on Patient Safety in Nigerian Public Hospitals

Abstract

Healthcare worker workload is an important organizational factor that may influence the quality and safety of healthcare delivery. Healthcare worker workload refers to the volume and intensity of clinical, administrative, documentation, communication, and other responsibilities assigned to healthcare workers within a specified period relative to the available workforce and working time. Patient safety refers to the prevention of avoidable harm, errors, adverse events, and unnecessary risks associated with healthcare delivery. In Nigerian public hospitals, high patient volumes, inadequate staffing, extended working hours, staff shortages, emergency demands, administrative responsibilities, and limited healthcare resources may increase the workload experienced by healthcare workers. Excessive workload may contribute to fatigue, reduced concentration, delayed clinical decisions, communication failures, incomplete documentation, medication errors, reduced adherence to safety procedures, and other conditions that may compromise patient safety. Conversely, manageable workloads may allow healthcare workers sufficient time to provide appropriate clinical attention, communicate effectively, monitor patients, document care, and comply with established safety procedures. Against this background, this study investigates the impact of healthcare worker workload on patient safety in Nigerian public hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, the Job Demands-Resources Model, and Systems Theory. Donabedian's Model provides a framework for understanding staffing levels and workload as structural and organizational factors that can influence healthcare processes and patient outcomes. The Job Demands-Resources Model explains how excessive job demands, including high patient volume, long working hours, emotional demands, and administrative responsibilities, may affect workers' capacity to perform effectively when adequate organizational resources are unavailable. Systems Theory views patient safety as an outcome of interactions among healthcare workers, staffing, organizational processes, equipment, communication, management systems, and patients. Collectively, these theoretical perspectives provide a suitable framework for explaining how healthcare worker workload may influence patient safety in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from selected Nigerian public hospitals using structured healthcare worker questionnaires, workload assessment tools, staffing records, duty rosters, patient-volume records, incident-reporting systems, and patient-safety assessment checklists. A multistage sampling technique will be employed to select states, public hospitals, departments, healthcare workers, and relevant patient-care units. Healthcare worker workload will be assessed using indicators such as patient-to-healthcare-worker ratio, number of patients managed per shift, working hours, overtime frequency, number of shifts, administrative workload, frequency of emergency assignments, workload intensity, and perceived workload. Where possible, objective staffing and patient-volume records will be used to complement self-reported workload measures. Patient safety will be assessed using indicators such as medication errors, patient falls, healthcare-associated infections, identification errors, documentation errors, adverse events, near misses, communication failures, compliance with patient-safety procedures, and incident-reporting patterns. Hospital incident records, quality-improvement reports, patient-safety registers, and clinical documentation will be reviewed where available to complement questionnaire data. Descriptive statistics will be used to summarize healthcare worker characteristics, workload levels, staffing patterns, and patient-safety indicators. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the impact of healthcare worker workload on patient safety. Diagnostic tests will also be conducted to assess the reliability of research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that excessive healthcare worker workload has a significant negative impact on patient safety in Nigerian public hospitals. Healthcare workers experiencing high patient volumes, long working hours, frequent overtime, and excessive administrative responsibilities are expected to report or demonstrate higher levels of patient-safety risks than workers operating under more manageable workloads. Excessive workload may contribute to fatigue, reduced attention, delayed response to patients' needs, communication failures, incomplete documentation, medication errors, and reduced compliance with infection prevention and other safety procedures. High workload may also limit healthcare workers' ability to monitor patients closely and identify early signs of clinical deterioration. Conversely, adequate staffing and manageable workloads are expected to improve healthcare workers' capacity to provide attentive, timely, and safe patient care. However, workload alone may not fully determine patient safety because safety is also influenced by staff competence, availability of medical equipment and medicines, organizational culture, communication systems, clinical protocols, supervision, and hospital infrastructure. Therefore, workload management should form part of a broader patient-safety improvement strategy. The study is expected to contribute to the literature on healthcare worker workload, patient safety, staffing, healthcare quality, occupational health, hospital administration, nursing, health-services management, and public health in Nigeria by providing empirical evidence on the relationship between workload and patient-safety outcomes. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospital administrators, nursing management, medical directors, human-resource departments, healthcare quality-improvement teams, professional regulatory bodies, development partners, and policymakers regarding strategies for managing healthcare worker workload. The study will also provide evidence-based recommendations for improving staffing levels, optimizing patient-to-worker ratios, monitoring workload indicators, reducing excessive overtime, improving duty-roster management, minimizing unnecessary administrative burdens, strengthening workforce planning, supporting healthcare workers during periods of high demand, and establishing workload and patient-safety monitoring systems in Nigerian public hospitals.
