Impact of Health Education on Early Detection of Prostate Cancer among Men in Nigeria
Abstract
Prostate cancer is an important public health concern among men and remains a significant cause of cancer-related morbidity and mortality in Nigeria. The disease may develop without obvious symptoms in its early stages, making awareness of risk factors, symptoms, appropriate evaluation, and timely healthcare-seeking behaviour important components of prostate cancer control. Early detection may increase opportunities for appropriate clinical assessment, diagnosis, treatment, and management before the disease progresses to advanced stages. Health education is an important public health intervention for improving men's knowledge of prostate cancer risk factors, warning signs, available diagnostic and screening options, and the importance of seeking medical evaluation when appropriate. In Nigeria, inadequate awareness, misconceptions about prostate cancer, fear of cancer diagnosis, stigma, financial constraints, limited access to healthcare services, and low perceived personal risk may contribute to delayed presentation and diagnosis. Health education programmes delivered through healthcare facilities, community outreach, workplaces, mass media, and other trusted communication channels may improve knowledge and encourage men to seek appropriate professional evaluation. Against this background, this study investigates the impact of health education on the early detection of prostate cancer among men in Nigeria. The study is anchored on the Health Belief Model (HBM), Theory of Planned Behavior (TPB), and Knowledge-Attitude-Practice (KAP) framework. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding how health education may influence men's decisions to seek prostate health evaluation. The Theory of Planned Behavior proposes that attitudes, subjective norms, and perceived behavioural control influence intentions and subsequent health-seeking behaviours. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes toward prostate cancer may influence appropriate healthcare-seeking and early detection practices. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how health education may contribute to earlier detection of prostate cancer among men in Nigeria. The study adopts a quantitative quasi-experimental research design. Primary data will be collected from adult men in selected communities, workplaces, and healthcare facilities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, communities, workplaces, healthcare facilities, and eligible participants. Where feasible, participants will be assigned to intervention and comparison groups. Men in the intervention group will receive structured health education covering prostate cancer risk factors, common signs and symptoms, the importance of early medical evaluation, available prostate health assessment procedures, and appropriate healthcare-seeking behaviour. Early detection will be assessed using indicators such as awareness of prostate cancer risk, timely presentation for evaluation when symptoms occur, participation in appropriate prostate health assessment, referral for further diagnostic evaluation where indicated, and identification of suspected or confirmed cases at an earlier stage. The study will not treat health education itself as a diagnostic test; clinical assessment and appropriate diagnostic procedures will be conducted by qualified healthcare professionals where applicable. Descriptive statistics will be used to summarize participants' sociodemographic characteristics, prostate cancer knowledge, health education exposure, and early-detection behaviours. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple logistic regression analysis, will be employed to determine the impact of health education on early detection-related outcomes. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and the robustness of the findings. The study anticipates that health education will have a significant positive impact on early detection of prostate cancer among men in Nigeria. Men who receive appropriate and accurate health education are expected to demonstrate improved knowledge of prostate cancer risk factors and warning signs and greater willingness to seek professional evaluation when appropriate. Health education may reduce misconceptions and fear, improve perceived susceptibility, encourage timely presentation, and increase awareness of available prostate health services. However, health education alone may not guarantee early detection because men may continue to face barriers such as cost of healthcare, limited availability of diagnostic services, distance to health facilities, fear of diagnosis, stigma, long waiting times, and limited access to qualified healthcare professionals. In addition, early detection strategies must be based on appropriate clinical guidance because screening and diagnostic decisions may vary according to age, risk factors, symptoms, and individual circumstances. Consequently, health education is expected to be most effective when linked to accessible, affordable, confidential, and quality-assured prostate health services. This study is expected to contribute to the literature on prostate cancer prevention, cancer awareness, men's health, health education, and public health in Nigeria by providing empirical evidence on the relationship between health education and early detection of prostate cancer among men. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, state ministries of health, hospitals, primary healthcare facilities, cancer control programmes, urology and oncology services, community health workers, men's health organizations, development partners, and policymakers regarding strategies for improving prostate cancer awareness and timely healthcare utilization. The study will also provide evidence-based recommendations for strengthening prostate cancer health education, addressing misconceptions and stigma, improving men's awareness of prostate health, promoting timely professional evaluation of concerning symptoms, strengthening referral systems, and expanding access to appropriate prostate cancer diagnostic services across Nigeria.
