Impact of Family Planning Education on Contraceptive Use among Women of Reproductive Age in Nigeria

Abstract

Family planning is an important component of reproductive health and contributes to the prevention of unintended pregnancies, reduction of maternal and neonatal health risks, improvement of birth spacing, and enhancement of women's reproductive autonomy. Contraceptive use enables women and couples to determine the timing and spacing of pregnancies and may include modern methods such as condoms, oral contraceptive pills, injectables, implants, intrauterine devices, emergency contraception, and permanent methods, as well as other fertility regulation approaches. Family planning education is an important public health intervention for providing women with accurate information about available contraceptive methods, effectiveness, benefits, possible side effects, eligibility, correct use, and sources of reproductive health services. In Nigeria, contraceptive use remains influenced by factors including inadequate knowledge, misconceptions about contraception, fear of side effects, cultural and religious beliefs, partner opposition, financial barriers, limited access to reproductive health services, and concerns about confidentiality and quality of care. Appropriate family planning education may improve women's knowledge, correct misconceptions, increase confidence in contraceptive decision-making, and encourage informed utilization of family planning services. Against this background, this study investigates the impact of family planning education on contraceptive use among women of reproductive age in Nigeria.

The study is anchored on the Health Belief Model (HBM), Theory of Planned Behavior (TPB), and Knowledge-Attitude-Practice (KAP) framework. The Health Belief Model explains preventive and health-related behaviours through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding women's decisions regarding contraceptive use. The Theory of Planned Behavior proposes that attitudes, subjective norms, and perceived behavioural control influence behavioural intentions and subsequent contraceptive practices. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from family planning education may influence contraceptive use. Collectively, these theoretical perspectives provide a comprehensive framework for explaining how family planning education may influence contraceptive use among women of reproductive age in Nigeria.

The study adopts a quantitative quasi-experimental research design. Primary data will be collected from women of reproductive age in selected communities and healthcare facilities in Nigeria using a structured interviewer-administered questionnaire. A multistage sampling technique will be employed to select states, local government areas, communities, healthcare facilities, and eligible participants. Where feasible, selected participants or healthcare facilities will be assigned to intervention and comparison groups. Women in the intervention group will receive structured family planning education covering fertility awareness, available contraceptive methods, effectiveness, correct use, possible side effects, management of common side effects, eligibility, dual protection, myths and misconceptions, and available family planning services. Contraceptive use will be assessed using indicators such as current use of contraception, type of contraceptive method used, consistency of use, duration of use, method switching, and utilization of family planning services. Descriptive statistics will be used to summarize participants' sociodemographic and reproductive characteristics, exposure to family planning education, and contraceptive-use patterns. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple logistic regression analysis, will be employed to determine the impact of family planning education on contraceptive use. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and the robustness of the findings.

The study anticipates that family planning education will have a significant positive impact on contraceptive use among women of reproductive age in Nigeria. Women exposed to structured family planning education are expected to demonstrate improved knowledge of contraceptive methods, greater understanding of their benefits and possible side effects, and increased likelihood of adopting appropriate contraceptive methods. Education may also correct misconceptions, reduce unnecessary fears concerning contraception, promote informed decision-making, and encourage women to seek professional family planning counselling. However, education alone may not guarantee contraceptive use because women may continue to experience barriers such as limited availability of preferred methods, cost of services or commodities, partner opposition, cultural expectations, fear of side effects, concerns about confidentiality, and limited access to trained healthcare providers. Consequently, family planning education is expected to be most effective when accompanied by accessible, affordable, confidential, respectful, and high-quality contraceptive services that provide women with a genuine choice of appropriate methods.

This study is expected to contribute to the literature on reproductive health, family planning, contraceptive use, maternal health, health education, and public health in Nigeria by providing empirical evidence on the impact of family planning education on contraceptive use among women of reproductive age. The findings will provide useful information for the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency (NPHCDA), state ministries of health, primary healthcare facilities, hospitals, reproductive health programmes, community health workers, development partners, and policymakers regarding strategies for improving contraceptive uptake. The study will also provide evidence-based recommendations for strengthening family planning education, integrating reproductive health counselling into routine healthcare services, addressing misconceptions and fears surrounding contraception, improving access to a broad range of contraceptive methods, strengthening community-based family planning programmes, and promoting informed and voluntary contraceptive decision-making among women across Nigeria.
