Impact of Falls Prevention Education on Fall-Prevention Practices among Older Adults in Nigeria

 

Abstract

Falls are an important public health concern among older adults and may result in fractures, head injuries, disability, loss of independence, reduced quality of life, hospitalization, and increased healthcare costs. The risk of falls may be influenced by factors such as poor balance, muscle weakness, visual impairment, unsafe footwear, medication-related effects, environmental hazards, and chronic health conditions. Despite the preventable nature of many falls, some older adults may have limited knowledge of fall-risk factors and appropriate preventive measures. Falls prevention education provides an opportunity to improve older adults' knowledge and encourage practical measures that reduce exposure to fall hazards. Against this background, this study investigates the impact of falls prevention education on fall-prevention practices among older adults in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Social Ecological Model. The Health Belief Model explains how older adults' perceptions of their susceptibility to falls, perceived severity of fall-related injuries, perceived benefits of preventive practices, perceived barriers, and cues to action may influence their adoption of fall-prevention behaviours. Social Cognitive Theory emphasizes observational learning, self-efficacy, behavioural reinforcement, and social support in the development and maintenance of preventive practices. The Social Ecological Model highlights the interaction between individual, family, community, healthcare, and environmental factors in influencing fall risk and prevention among older adults. Collectively, these theoretical perspectives provide a suitable framework for explaining how falls prevention education may influence fall-prevention practices among older adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 60 years and above residing in selected urban and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, and eligible older adults. Falls prevention education will be assessed using indicators such as exposure to structured falls prevention education, frequency and duration of educational sessions, information on fall-risk factors, balance and strength exercises, safe footwear, medication safety, visual health, environmental hazard identification, safe movement techniques, use of mobility aids, and appropriate healthcare-seeking following falls or near-falls. Fall-prevention practices will be assessed using indicators such as removal of household hazards, adequate lighting, use of handrails and support structures, appropriate footwear, safe movement around the home and community, adherence to recommended exercises, appropriate use of walking aids, regular vision assessment, medication review where indicated, and avoidance of risky activities. Data will be collected using structured questionnaires, validated falls knowledge and prevention practice instruments, home safety assessment checklists, health education attendance records, and relevant healthcare records where available. Descriptive statistics will be used to summarize participants' characteristics, exposure to falls prevention education, and fall-prevention practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of falls prevention education on fall-prevention practices. Where a quasi-experimental design is adopted, fall-prevention practice scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that falls prevention education has a significant positive impact on fall-prevention practices among older adults in Nigeria. Older adults who receive structured and practical falls prevention education are expected to demonstrate improved awareness and adoption of measures such as maintaining safer home environments, using appropriate footwear, participating in recommended balance and strengthening exercises, using mobility aids correctly, and seeking appropriate assessment for visual or medication-related fall risks. Practical demonstrations, home-based education, caregiver involvement, and repeated reinforcement may improve older adults' confidence and ability to incorporate fall-prevention measures into their daily routines. Education may also encourage older adults and their families to recognize falls as preventable health events rather than an unavoidable consequence of ageing. However, physical limitations, financial constraints, unsafe housing environments, inadequate community support, low health literacy, limited access to physiotherapy and other healthcare services, and cultural beliefs may affect the adoption of recommended practices. The study therefore expects accessible, practical, culturally appropriate, and regularly reinforced falls prevention education to contribute significantly to improved fall-prevention practices among older adults in Nigeria. The study is expected to contribute to the literature on falls prevention education, fall-prevention practices, older adult health, injury prevention, healthy ageing, geriatric care, community health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Senior Citizens Centre, National Primary Health Care Development Agency, state ministries and agencies responsible for health and social welfare, primary healthcare centres, physiotherapists, occupational therapists, nurses, community health workers, caregivers, development partners, and policymakers regarding strategies for preventing falls among older adults. The study will also provide evidence-based recommendations for integrating falls prevention education into routine elderly healthcare, strengthening home safety assessment and counselling, promoting balance and strength exercises, improving access to mobility and assistive devices, training healthcare workers in falls prevention, and involving families and caregivers in fall-risk reduction programmes across Nigeria.
