Impact of Antenatal Blood Pressure Monitoring on Early Detection of Pre-eclampsia among Pregnant Women in Nigeria

Abstract

Pre-eclampsia remains an important cause of maternal and perinatal morbidity and mortality in Nigeria and is characterized by new-onset hypertension during pregnancy, often accompanied by proteinuria or other signs of maternal organ dysfunction. If not detected and managed promptly, pre-eclampsia may progress to severe complications, including eclampsia, stroke, placental abruption, preterm birth, fetal growth restriction, and maternal or neonatal death. Routine blood pressure monitoring during antenatal care provides an important opportunity for early identification of abnormal blood pressure patterns and timely clinical assessment of pregnant women at increased risk of pre-eclampsia. However, inconsistent antenatal attendance, inadequate blood pressure monitoring, shortages of trained healthcare personnel, limited availability or poor maintenance of blood pressure equipment, inadequate documentation, and delayed referral may reduce the effectiveness of antenatal hypertension surveillance in Nigeria. Against this background, this study investigates the impact of antenatal blood pressure monitoring on early detection of pre-eclampsia among pregnant women in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how pregnant women's perceptions of susceptibility to pre-eclampsia, perceived severity, perceived benefits of antenatal monitoring, perceived barriers, and cues to action may influence antenatal care attendance and participation in blood pressure monitoring. The Social Ecological Model emphasizes the influence of individual, household, community, healthcare facility, socioeconomic, and policy factors on antenatal care utilization and detection of pregnancy complications. The Health Systems Framework emphasizes service delivery, health workforce, medical equipment, health information systems, financing, governance, and accessibility as essential components of effective maternal health services. Collectively, these theoretical perspectives provide a suitable framework for explaining how antenatal blood pressure monitoring may influence early detection of pre-eclampsia in Nigeria. The study will adopt a quantitative analytical cohort or retrospective cohort research design. The study population will comprise pregnant women receiving antenatal care in selected healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, antenatal clinics, and eligible pregnant women. Antenatal blood pressure monitoring will be assessed using indicators such as frequency of blood pressure measurement, timing of measurements during antenatal visits, availability and functionality of blood pressure equipment, appropriate measurement procedures, documentation of blood pressure readings, identification of elevated blood pressure, repeat measurements, healthcare worker response to abnormal readings, and referral for further assessment. Early detection of pre-eclampsia will be assessed using indicators such as identification of new-onset hypertension, documented proteinuria or other relevant clinical features, gestational age at detection, time from abnormal blood pressure reading to clinical assessment, confirmed diagnosis, referral, and initiation of appropriate management. Data will be collected using structured questionnaires, antenatal care records, maternal medical records, blood pressure monitoring charts, laboratory records, referral registers, and hospital maternity records. Descriptive statistics will be used to summarize participants' characteristics, antenatal attendance, blood pressure monitoring patterns, and pre-eclampsia detection outcomes. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of antenatal blood pressure monitoring on early detection of pre-eclampsia. Where a cohort design is adopted, the timing and rate of pre-eclampsia detection among women receiving consistent blood pressure monitoring may be compared with those among women receiving inadequate monitoring, while relevant maternal, socioeconomic, and pregnancy-related factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that consistent antenatal blood pressure monitoring has a significant positive impact on the early detection of pre-eclampsia among pregnant women in Nigeria. Women whose blood pressure is routinely and appropriately monitored during antenatal visits are expected to have a higher likelihood of early identification of abnormal blood pressure and pre-eclampsia compared with women receiving inadequate monitoring. Early detection may facilitate timely laboratory assessment, closer maternal and fetal monitoring, appropriate treatment, referral to higher-level facilities, and other measures aimed at preventing progression to severe disease. However, inadequate antenatal attendance, limited availability of functional blood pressure equipment, poor measurement techniques, insufficient documentation, shortage of skilled healthcare personnel, and delayed referral may reduce the effectiveness of monitoring. The study therefore expects systematic and quality-assured antenatal blood pressure monitoring, combined with timely clinical assessment and appropriate referral, to contribute significantly to early detection and management of pre-eclampsia in Nigeria. The study is expected to contribute to the literature on antenatal blood pressure monitoring, pre-eclampsia detection, maternal health, hypertensive disorders of pregnancy, antenatal care, maternal morbidity, maternal mortality, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, obstetricians, midwives, nurses, community health workers, maternal health programme managers, development partners, and policymakers regarding strategies for strengthening antenatal hypertension screening. The study will also provide evidence-based recommendations for improving routine blood pressure monitoring during antenatal care, ensuring availability and maintenance of appropriate equipment, strengthening healthcare worker training, improving documentation and clinical response to abnormal readings, strengthening referral systems, promoting early antenatal care attendance, and integrating systematic hypertension surveillance into broader maternal and newborn health programmes across Nigeria.
