Effect of Stroke Rehabilitation Education on Rehabilitation Knowledge among Stroke Survivors in Nigeria

Abstract

Stroke is a major public health concern and an important cause of long-term disability among adults in Nigeria. Stroke survivors may experience physical, cognitive, communication, emotional, and functional difficulties that can affect their independence, quality of life, and ability to participate in daily activities. Rehabilitation plays an important role in helping survivors regain or maintain functional abilities and adapt to limitations following stroke. However, inadequate knowledge of rehabilitation principles, limited access to rehabilitation professionals, financial barriers, transportation difficulties, and insufficient patient and caregiver education may affect rehabilitation participation and outcomes. Stroke rehabilitation education provides an opportunity to improve survivors' understanding of rehabilitation goals, exercises, assistive devices, medication adherence, prevention of recurrent stroke, and appropriate self-management. Against this background, this study investigates the effect of stroke rehabilitation education on rehabilitation knowledge among stroke survivors in Nigeria. The study will be anchored on the Health Belief Model, Self-Efficacy Theory, and Social Cognitive Theory. The Health Belief Model explains how stroke survivors' perceptions of the severity of stroke-related disability, perceived benefits of rehabilitation, perceived barriers, and cues to action may influence their understanding and participation in rehabilitation. Self-Efficacy Theory emphasizes survivors' confidence in their ability to understand and perform recommended rehabilitation activities despite physical and environmental limitations. Social Cognitive Theory emphasizes observational learning, behavioural reinforcement, social support, and interaction between individuals and their rehabilitation environment. Collectively, these theoretical perspectives provide a suitable framework for explaining how stroke rehabilitation education may influence rehabilitation knowledge among stroke survivors in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adult stroke survivors receiving care or rehabilitation services in selected hospitals, rehabilitation centres, and physiotherapy clinics across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, healthcare facilities, rehabilitation centres, and eligible stroke survivors. Stroke rehabilitation education will be assessed using indicators such as exposure to structured rehabilitation education, frequency and duration of educational sessions, information on rehabilitation goals, prescribed exercises, mobility and balance training, activities of daily living, positioning, prevention of complications, use of assistive devices, medication adherence, prevention of recurrent stroke, nutrition, communication support, and follow-up rehabilitation services. Rehabilitation knowledge will be assessed using indicators such as understanding of rehabilitation goals, appropriate exercise principles, mobility and positioning, prevention of contractures and falls, safe use of assistive devices, activities of daily living, medication adherence, recognition of complications, prevention of recurrent stroke, importance of continued rehabilitation, and appropriate follow-up care. Data will be collected using structured questionnaires, validated stroke rehabilitation knowledge instruments, patient education records, rehabilitation attendance records, and relevant clinical documents. Descriptive statistics will be used to summarize participants' characteristics, exposure to rehabilitation education, and levels of rehabilitation knowledge. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of stroke rehabilitation education on rehabilitation knowledge. Where a quasi-experimental design is adopted, rehabilitation knowledge scores before and after the educational intervention may be compared to determine changes associated with the programme. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that stroke rehabilitation education has a significant positive effect on rehabilitation knowledge among stroke survivors in Nigeria. Stroke survivors who receive structured and practical rehabilitation education are expected to demonstrate greater knowledge of rehabilitation goals, appropriate exercises, safe mobility, positioning, prevention of complications, use of assistive devices, medication adherence, and prevention of recurrent stroke than survivors with limited exposure to rehabilitation education. Practical demonstrations, individualized counselling, visual educational materials, and caregiver involvement may improve understanding and retention of rehabilitation information. Repeated education during follow-up may further reinforce knowledge and support continued participation in rehabilitation. However, cognitive and communication difficulties, severity of disability, limited access to rehabilitation professionals, financial constraints, transportation challenges, low health literacy, and inadequate rehabilitation facilities may affect the effectiveness of rehabilitation education. The study therefore expects accessible, individualized, practical, and regularly reinforced stroke rehabilitation education to contribute significantly to improved rehabilitation knowledge among stroke survivors in Nigeria. The study is expected to contribute to the literature on stroke rehabilitation education, rehabilitation knowledge, stroke disability management, stroke recovery, patient education, self-management, physiotherapy, occupational therapy, neurological rehabilitation, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospitals, rehabilitation centres, physiotherapists, occupational therapists, speech and language therapists, nurses, physicians, caregivers, stroke support organizations, development partners, and policymakers regarding strategies for improving rehabilitation outcomes. The study will also provide evidence-based recommendations for integrating structured rehabilitation education into stroke care, strengthening patient and caregiver counselling, improving access to rehabilitation professionals and services, developing accessible educational materials for stroke survivors, strengthening follow-up rehabilitation programmes, and promoting continuity of rehabilitation care across Nigeria.
