Effect of School Health Services on Health Screening among Secondary School Students in Nigeria
Abstract
School health services provide an important platform for promoting the health and well-being of children and adolescents by bringing preventive and basic healthcare activities closer to students within the school environment. These services may include health screening, health assessment, health education, first aid, referral services, immunization support, counselling, and other appropriate preventive healthcare activities. Health screening refers to the systematic assessment of students to identify health conditions or risk factors that may not yet have produced obvious symptoms, including vision problems, hearing difficulties, dental conditions, nutritional problems, hypertension, and other selected health concerns. Early identification of such conditions can facilitate timely referral, treatment, and appropriate support, thereby reducing their potential effects on students' academic performance, school attendance, and general well-being. In Nigeria, many secondary school students may have limited access to routine preventive healthcare because of financial constraints, parental availability, geographical barriers, inadequate health facilities, and limited awareness of preventive health services. School health services may provide a convenient avenue for improving access to health screening because students can receive preventive services within an environment they regularly attend. Against this background, this study investigates the effect of school health services on health screening among secondary school students in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Andersen's Behavioral Model of Health Services Use. The Health Belief Model explains participation in health screening through perceived susceptibility to health problems, perceived severity, perceived benefits of screening, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the influence of knowledge, self-efficacy, social environment, peer influence, and institutional support on health-related behaviour. Andersen's Behavioral Model provides a framework for understanding healthcare utilization in relation to predisposing characteristics, enabling resources, and healthcare needs, with school-based health services representing an important enabling resource for students. Collectively, these theoretical perspectives provide a suitable framework for explaining how school health services may influence health screening among secondary school students in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from students in selected secondary schools in Nigeria using structured questionnaires, student health-screening records, and appropriate screening assessment tools. A multistage sampling technique will be employed to select states, local government areas, schools, classes, and eligible students. Where feasible, selected schools will be assigned to intervention and comparison groups. Students in intervention schools will receive structured school health services involving organized health screening, health education, basic health assessment, counselling, referral, and appropriate follow-up. Health screening may include assessments such as visual acuity, hearing, oral health, body mass index, blood pressure, nutritional status, and other age-appropriate health assessments depending on available resources and ethical approval. Health screening will be assessed using indicators such as participation in screening, completion of recommended screening assessments, identification of previously undetected health conditions or risk factors, referral following abnormal findings, and attendance at follow-up services where applicable. Additional information will be collected on students' sociodemographic characteristics, previous exposure to school health services, parental health-seeking practices, school characteristics, and access to healthcare outside the school environment. Descriptive statistics will be used to summarize students' characteristics, availability and utilization of school health services, screening participation, and identified health conditions. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the effect of school health services on health screening. Where appropriate, screening participation and detection outcomes before and after implementation of school health services will be compared. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that school health services have a significant positive effect on health screening among secondary school students in Nigeria. Students attending schools with organized and accessible school health services are expected to demonstrate higher participation in routine health screening and greater identification of previously unrecognized health conditions or risk factors. School-based screening is expected to reduce barriers associated with accessing health facilities outside school, improve students' exposure to preventive healthcare, and facilitate early referral for students requiring further assessment or treatment. Health education provided alongside screening is also expected to improve students' awareness of the importance of preventive healthcare and encourage appropriate health-seeking behaviour. However, the effectiveness of school health services may depend on the availability of trained health personnel, screening equipment, school management support, parental consent and involvement, referral mechanisms, and access to appropriate healthcare facilities. Consequently, school health services should be implemented as a comprehensive preventive-health programme rather than as isolated screening activities. The study is expected to contribute to the literature on school health services, health screening, adolescent health, preventive healthcare, school-based healthcare, health education, early detection, and public health in Nigeria by providing empirical evidence on the role of school health services in improving health screening among secondary school students. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, school management authorities, primary healthcare agencies, school health coordinators, healthcare professionals, public health practitioners, parents, development partners, and policymakers regarding strategies for strengthening school-based preventive healthcare. The study will also provide evidence-based recommendations for establishing or strengthening school health programmes, conducting regular age-appropriate health screening, improving referral and follow-up systems, providing adequate screening equipment and trained personnel, strengthening collaboration between schools and healthcare facilities, and promoting preventive health among secondary school students in Nigeria.
