Effect of Primary Healthcare Facility Equipment Availability on Patient Satisfaction in Nigeria
Abstract
The availability of appropriate healthcare equipment is an important component of effective primary healthcare delivery because healthcare providers require functional equipment to conduct diagnosis, monitoring, treatment, screening, and other essential health services. Primary healthcare facility equipment availability refers to the presence, adequacy, functionality, accessibility, and regular maintenance of essential medical and diagnostic equipment required for the delivery of appropriate healthcare services at primary healthcare facilities. Patient satisfaction refers to the extent to which patients' expectations and perceptions regarding the healthcare services they receive are met, including satisfaction with service quality, diagnostic processes, provider interaction, waiting time, facility environment, availability of services, and overall healthcare experience. In Nigeria, some primary healthcare facilities may experience inadequate, outdated, poorly maintained, or non-functional equipment, potentially limiting the range and quality of services that healthcare providers can deliver. When essential equipment is unavailable or dysfunctional, patients may be referred to other facilities, experience delays in diagnosis and treatment, incur additional transportation and healthcare costs, or perceive the facility as unable to meet their healthcare needs. These experiences may negatively affect patient satisfaction and confidence in primary healthcare services. Conversely, adequate availability of functional equipment may improve service efficiency, diagnostic capacity, provider confidence, and patients' perceptions of healthcare quality. Against this background, this study investigates the effect of primary healthcare facility equipment availability on patient satisfaction in Nigeria. The study is anchored on Donabedian's Structure-Process-Outcome Model, Expectancy-Disconfirmation Theory, and Andersen's Behavioral Model of Health Services Use. Donabedian's Model identifies healthcare equipment as an important structural component that can influence healthcare delivery processes and patient outcomes. Expectancy-Disconfirmation Theory explains patient satisfaction as the result of comparing patients' expectations of healthcare services with their actual experiences. Andersen's Behavioral Model explains healthcare utilization and healthcare experiences in relation to predisposing characteristics, enabling resources, and healthcare needs, with facility equipment representing an important health-system resource. Collectively, these theoretical perspectives provide a suitable framework for explaining how equipment availability may influence patient satisfaction in Nigerian primary healthcare facilities. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from patients attending selected primary healthcare facilities in Nigeria using a structured patient questionnaire and a standardized healthcare facility equipment assessment checklist. A multistage sampling technique will be employed to select states, local government areas, primary healthcare facilities, service units, and eligible patients. Primary healthcare facility equipment availability will be assessed using indicators such as availability of essential medical equipment, functionality of equipment, adequacy of equipment relative to patient volume, availability of diagnostic equipment, availability of monitoring equipment, equipment accessibility to healthcare workers, equipment maintenance, and frequency of equipment breakdown. Where feasible, direct facility observation, equipment inventories, maintenance records, and facility registers will be used to objectively assess equipment availability and functionality. Patient satisfaction will be assessed using indicators such as satisfaction with diagnostic services, quality of care, provider interaction, waiting time, availability of healthcare services, facility environment, confidence in the facility, and overall satisfaction. Additional information will be collected on patients' sociodemographic characteristics, type of healthcare service received, frequency of facility visits, distance travelled, healthcare costs, and previous experiences with the facility. Descriptive statistics will be used to summarize facility characteristics, equipment availability, patient characteristics, and satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of equipment availability on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that adequate availability of functional primary healthcare facility equipment has a significant positive effect on patient satisfaction in Nigeria. Patients attending facilities with adequate and functional equipment are expected to report higher satisfaction with healthcare services because appropriate equipment can improve diagnostic capacity, reduce unnecessary referrals, facilitate timely treatment, and strengthen confidence in the quality of care provided. Availability of basic diagnostic and monitoring equipment is particularly expected to improve patients' perceptions of service effectiveness and responsiveness. Conversely, frequent equipment shortages, malfunctioning equipment, and inadequate maintenance may result in service delays, referrals to other facilities, additional costs, and dissatisfaction. However, equipment availability alone may not fully determine patient satisfaction because patients' experiences are also influenced by healthcare worker attitudes, waiting time, medicine availability, cleanliness, cost of services, accessibility, and perceived treatment outcomes. Therefore, improvements in equipment availability should form part of a broader strategy for strengthening primary healthcare quality. The study is expected to contribute to the literature on healthcare equipment availability, patient satisfaction, primary healthcare quality, healthcare infrastructure, health-services delivery, healthcare management, and public health in Nigeria by providing empirical evidence on the relationship between facility equipment and patients' experiences. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, state primary healthcare development agencies, local government health authorities, primary healthcare facility managers, healthcare providers, development partners, and policymakers regarding strategies for improving primary healthcare infrastructure. The study will also provide evidence-based recommendations for strengthening healthcare equipment procurement, distribution, maintenance, functionality monitoring, equipment inventories, facility-level needs assessments, and resource allocation, while ensuring that primary healthcare facilities have appropriate and functional equipment to provide satisfactory and effective healthcare services to patients in Nigeria.
