Effect of Outbreak Preparedness Training on Emergency Response Capacity among Healthcare Workers in Nigeria
Abstract
Outbreak preparedness is an important component of public health emergency management because healthcare workers are often among the first professionals responsible for identifying suspected cases, initiating infection prevention and control measures, reporting unusual health events, providing appropriate clinical care, and supporting coordinated emergency responses. Outbreak preparedness training refers to structured educational, practical, simulation-based, and competency-development activities designed to equip healthcare workers with the knowledge and skills required to prevent, identify, report, investigate, and respond effectively to disease outbreaks and other public health emergencies. Emergency response capacity refers to the ability of healthcare workers to rapidly and effectively perform the knowledge, skills, coordination, communication, surveillance, infection prevention, case management, referral, and other response functions required during an emergency. In Nigeria, recurrent infectious disease outbreaks and other public health emergencies place considerable demands on healthcare workers across different levels of the health system. However, inadequate training, limited refresher programmes, insufficient simulation exercises, shortages of emergency-response resources, weak communication systems, and limited familiarity with outbreak-response protocols may constrain healthcare workers' preparedness and response capacity. Regular and competency-based outbreak preparedness training may strengthen healthcare workers' ability to recognize emergencies, activate response procedures, implement infection prevention measures, communicate effectively, report suspected cases, and coordinate with relevant public health authorities. Against this background, this study investigates the effect of outbreak preparedness training on emergency response capacity among healthcare workers in Nigeria. The study is anchored on Human Capital Theory, the Health Systems Framework, and the PRECEDE-PROCEED Model. Human Capital Theory views training as an investment in knowledge and skills that improves individual competence and organizational performance. The Health Systems Framework recognizes the health workforce as a fundamental component of a functional health system and emphasizes workforce capacity as essential for effective service delivery and emergency preparedness. The PRECEDE-PROCEED Model provides a framework for understanding how knowledge, skills, enabling resources, and organizational conditions influence health-related practices and preparedness behaviours. Collectively, these theoretical perspectives provide a suitable framework for explaining how outbreak preparedness training may influence emergency response capacity among healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental research design. Healthcare workers will be selected from participating hospitals and primary healthcare facilities and assessed before and after exposure to a structured outbreak preparedness training programme. A multistage sampling technique will be employed to select states, healthcare facilities, professional categories, and eligible healthcare workers. Outbreak preparedness training will cover disease outbreak recognition, emergency notification procedures, public health surveillance, infection prevention and control, personal protective equipment, specimen handling, case identification, basic outbreak investigation, isolation procedures, risk communication, contact tracing, referral procedures, emergency coordination, and appropriate use of emergency-response protocols. Training exposure will be assessed using indicators such as participation in outbreak preparedness training, training frequency, training duration, recency of training, practical exercises, simulation drills, relevance of training content, and participation in refresher programmes. Emergency response capacity will be assessed using indicators such as knowledge of emergency protocols, ability to recognize suspected outbreaks, timeliness of emergency notification, infection prevention and control skills, correct use of personal protective equipment, communication and coordination skills, case management preparedness, referral capacity, surveillance and reporting skills, emergency decision-making, participation in simulation exercises, and ability to implement facility emergency-response procedures. Pre-training and post-training assessments will be compared to determine changes in emergency response capacity. Descriptive statistics will be used to summarize participants' characteristics, training exposure, and emergency-response capacity. Inferential statistical techniques, including paired-sample tests, chi-square tests, correlation analysis, and regression analysis, will be used to determine the effect of outbreak preparedness training on emergency response capacity. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that outbreak preparedness training has a significant positive effect on emergency response capacity among healthcare workers in Nigeria. Healthcare workers who receive structured and practical outbreak preparedness training are expected to demonstrate improved knowledge, technical skills, confidence, and ability to perform essential emergency-response functions. Training is expected to improve recognition and notification of suspected outbreaks, infection prevention and control practices, appropriate use of personal protective equipment, emergency communication, case management, referral procedures, surveillance activities, and coordination with public health authorities. Simulation exercises and practical demonstrations are also expected to strengthen the ability of healthcare workers to apply emergency protocols under realistic conditions. Conversely, healthcare workers with limited or outdated training may demonstrate gaps in emergency procedures, delayed reporting, inappropriate infection prevention practices, and reduced readiness to respond effectively to public health emergencies. However, training alone may not fully determine emergency response capacity because effective response also depends on the availability of personal protective equipment, medicines, laboratory services, emergency funding, transportation, communication systems, staffing, leadership, infrastructure, and functional emergency-response plans. Therefore, training should be combined with adequate resources and institutional preparedness measures. The study is expected to contribute to the literature on outbreak preparedness training, emergency response capacity, healthcare worker competency, public health emergency preparedness, infectious disease surveillance, infection prevention and control, health security, and health systems in Nigeria by providing empirical evidence on the role of workforce training in strengthening emergency response. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, National Primary Health Care Development Agency, state ministries of health, state epidemiology units, healthcare institutions, professional health bodies, development partners, emergency-response organizations, and policymakers regarding strategies for improving healthcare worker preparedness. The study will also provide evidence-based recommendations for establishing regular outbreak preparedness training, strengthening refresher courses, expanding simulation and emergency drills, improving competency-based assessments, strengthening infection prevention and control training, improving emergency communication and reporting systems, ensuring adequate response resources, and integrating outbreak preparedness competencies into continuing professional development programmes for healthcare workers across Nigeria.
