Effect of Mental Health Education on Mental Health Service Utilization among University Students in Nigeria

Abstract

Mental health is an important component of overall health and well-being, particularly among university students who may experience academic pressure, financial difficulties, social challenges, relationship problems, and other stressors associated with young adulthood and higher education. Mental health difficulties that are not recognized or appropriately managed may negatively affect students' academic performance, social functioning, quality of life, and general well-being. Mental health education provides students with information about common mental health conditions, symptoms, risk factors, coping strategies, available mental health services, the importance of early help-seeking, confidentiality, and ways of supporting individuals experiencing mental health difficulties. Mental health service utilization refers to the use of appropriate professional or institutional mental health services, including counselling, psychological support, psychiatric services, student health services, and other qualified mental health resources. In Nigeria, university students may underutilize mental health services because of inadequate awareness, stigma, misconceptions about mental illness, concerns about confidentiality, perceived lack of need, financial barriers, limited availability of services, and uncertainty about where to seek help. Effective mental health education may improve students' knowledge, reduce misconceptions and stigma, increase awareness of available services, and encourage appropriate utilization of mental health support. Against this background, this study investigates the effect of mental health education on mental health service utilization among university students in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains health service utilization through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding students' decisions to seek professional mental health support. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping health-seeking behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from mental health education may influence mental health service utilization. Collectively, these theoretical perspectives provide a suitable framework for explaining how mental health education may influence mental health service utilization among university students in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from undergraduate students in selected universities in Nigeria using a structured anonymous self-administered questionnaire. A multistage sampling technique will be employed to select states, universities, faculties or departments, academic levels, and eligible students. Where feasible, selected student groups will be assigned to intervention and comparison groups. Students in the intervention group will receive structured mental health education covering common mental health conditions, warning signs, risk factors, healthy coping strategies, available mental health services, benefits of early help-seeking, confidentiality, stigma reduction, and appropriate pathways for accessing professional mental health support. Mental health service utilization will be assessed using indicators such as awareness and use of university counselling services, utilization of professional psychological or psychiatric services, consultation with qualified mental health professionals, attendance at mental health support sessions, and appropriate referral or follow-up where applicable. Descriptive statistics will be used to summarize students' sociodemographic characteristics, mental health knowledge, exposure to education, and mental health service utilization. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the effect of mental health education on mental health service utilization. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that mental health education has a significant positive effect on mental health service utilization among university students in Nigeria. Students exposed to structured mental health education are expected to demonstrate improved knowledge of mental health conditions and greater awareness and utilization of appropriate mental health services. Education may reduce misconceptions and stigma, improve recognition of mental health difficulties, increase awareness of available counselling and professional services, strengthen confidence in seeking help, and encourage earlier utilization of appropriate support. However, education alone may not guarantee increased service utilization because students may continue to face barriers such as stigma, concerns about confidentiality, limited availability of qualified professionals, cost, long waiting times, perceived lack of need, and fear of being judged. Consequently, mental health education is expected to be most effective when supported by accessible and confidential university-based mental health services, trained counsellors, referral systems, anti-stigma programmes, and supportive institutional policies. The study is expected to contribute to the literature on mental health education, mental health service utilization, university student health, health-seeking behaviour, psychological well-being, health promotion, and public health in Nigeria by providing empirical evidence on the effect of mental health education on mental health service utilization among university students. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Universities Commission, university administrations, university health centres, counselling units, mental health professionals, state ministries of health, public health practitioners, development partners, and policymakers regarding strategies for improving access to and utilization of mental health services among young adults. The study will also provide evidence-based recommendations for strengthening university mental health education programmes, improving availability and accessibility of confidential counselling services, reducing mental health stigma, strengthening referral systems, promoting early help-seeking, and improving mental health support for university students across Nigeria.
