Effect of Lymphatic Filariasis Mass Drug Administration on Lymphatic Filariasis Prevalence in Endemic Communities in Nigeria
Abstract
Lymphatic filariasis (LF) is a neglected tropical disease caused by filarial parasites and remains a public health concern in some endemic communities in Nigeria. The disease is transmitted by mosquitoes and can result in chronic conditions such as lymphoedema and hydrocele, which may cause disability, social stigma, reduced productivity, and substantial social and economic consequences for affected individuals and households. Persistent transmission in endemic communities is associated with repeated exposure to infected mosquitoes and the continued presence of infected individuals within affected populations. Mass Drug Administration (MDA) is a major public health strategy for controlling and eliminating lymphatic filariasis and involves the systematic distribution of recommended antifilarial medicines to eligible populations in endemic areas, irrespective of individual infection status. The objective of MDA is to reduce the level of infection in the population and interrupt transmission. Lymphatic filariasis prevalence refers to the proportion of individuals in a defined population who are infected with or affected by LF at a particular time. In Nigeria, sustained and adequately implemented MDA programmes are important for reducing LF transmission and supporting national elimination objectives. Against this background, this study investigates the effect of lymphatic filariasis mass drug administration on lymphatic filariasis prevalence in endemic communities in Nigeria. The study will be anchored on the Health Belief Model, the Health Systems Framework, and the Social Ecological Model. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action and provides a framework for understanding community participation in MDA. The Health Systems Framework emphasizes service delivery, health workforce, health information, essential medicines, financing, and accessibility as important components of effective disease-control programmes. The Social Ecological Model recognizes that disease transmission and preventive health behaviour are influenced by interacting individual, household, community, environmental, and health-system factors. Collectively, these theoretical perspectives provide a suitable framework for explaining how LF MDA may influence lymphatic filariasis prevalence in endemic Nigerian communities. The study will adopt a quantitative quasi-experimental, cohort, or repeated cross-sectional research design, depending on the availability of programme and surveillance data. Data will be collected from residents of selected communities identified as endemic for lymphatic filariasis in Nigeria. A multistage sampling technique will be used to select states, local government areas, endemic communities, households, and eligible participants. LF MDA implementation will be assessed using indicators such as MDA coverage, number of treatment rounds received, frequency of MDA, proportion of eligible residents receiving treatment, treatment adherence, availability of medicines, community mobilization, health education, availability of trained drug distributors, supervision, and programme continuity. LF prevalence will be assessed using appropriate parasitological or approved diagnostic methods and relevant programme surveillance indicators to determine the proportion of residents with evidence of LF infection. Data will be obtained from structured questionnaires, MDA registers, community drug-distributor records, programme reports, health facility records, and relevant laboratory or surveillance data where available. Descriptive statistics will be used to summarize participants' characteristics, MDA coverage, and LF prevalence patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, incidence or prevalence comparisons, and regression analysis, will be used to determine the effect of MDA on LF prevalence. Where appropriate, changes in prevalence between baseline and subsequent assessment periods will be examined. Diagnostic tests will also be conducted to assess the reliability and robustness of the findings. The study is expected to find that effective lymphatic filariasis MDA has a significant negative effect on LF prevalence in endemic communities in Nigeria. Communities with high and sustained MDA coverage are expected to demonstrate lower levels of LF infection than communities with inadequate or inconsistent programme coverage. Repeated administration of recommended antifilarial medicines may reduce the number of infected individuals capable of contributing to transmission and progressively lower infection levels within endemic populations. Effective community mobilization and health education may increase participation in MDA and improve programme coverage. Conversely, low treatment coverage, refusal to take medicines, inadequate community awareness, medicine shortages, weak supervision, poor programme monitoring, and difficulties reaching remote populations may reduce the effectiveness of MDA and allow transmission to persist. However, MDA alone may not completely determine LF prevalence because mosquito transmission patterns, environmental conditions, population mobility, individual participation, and other vector-control measures may also influence infection levels. The study is expected to contribute to the literature on lymphatic filariasis MDA, LF prevalence, neglected tropical disease elimination, community health, infectious disease control, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, neglected tropical disease programme authorities, National Primary Health Care Development Agency, state ministries of health, local government health authorities, community health workers, development partners, and policymakers regarding strategies for strengthening LF elimination programmes. The study will also provide evidence-based recommendations for improving MDA coverage, strengthening community mobilization, ensuring uninterrupted availability of antifilarial medicines, improving drug-distributor training and supervision, strengthening programme monitoring and surveillance, improving identification of populations missed during MDA, and integrating MDA with other neglected tropical disease control and vector-control interventions in endemic communities across Nigeria.
