Effect of Healthcare Worker Communication Skills on Patient Satisfaction in Nigerian Public Hospitals
Abstract
Healthcare worker communication is an essential component of effective patient-centred care because clear, respectful, timely, and understandable communication can influence patients' understanding of their health conditions, confidence in healthcare providers, participation in treatment decisions, and overall experience of healthcare services. Healthcare worker communication skills refer to the ability of healthcare professionals to communicate effectively with patients through verbal, non-verbal, interpersonal, listening, information-sharing, questioning, explanation, and counselling skills. Patient satisfaction refers to the extent to which patients' expectations and perceptions regarding healthcare services and their overall hospital experience are met. In Nigerian public hospitals, challenges such as overcrowding, high patient volumes, heavy workloads, staff shortages, limited consultation time, language differences, and inadequate communication training may affect the quality of interactions between healthcare workers and patients. Poor communication may result in misunderstanding, inadequate information, reduced trust, anxiety, dissatisfaction, and reluctance to follow healthcare recommendations. Conversely, effective communication may improve patients' understanding, confidence, dignity, trust, and perception of service quality. Against this background, this study investigates the effect of healthcare worker communication skills on patient satisfaction in Nigerian public hospitals. The study is anchored on Donabedian's Structure-Process-Outcome Model, Patient-Centred Care Theory, and Expectancy-Disconfirmation Theory. Donabedian's Model identifies healthcare worker practices and interactions as important components of the healthcare process that can influence patient outcomes and perceptions of care quality. Patient-Centred Care Theory emphasizes respectful communication, shared understanding, patient participation, responsiveness, and consideration of patients' preferences and needs as essential elements of quality healthcare. Expectancy-Disconfirmation Theory explains patient satisfaction as the result of comparing patients' expectations of healthcare services with their actual experiences. Collectively, these theoretical perspectives provide a suitable framework for explaining how healthcare worker communication skills may influence patient satisfaction in Nigerian public hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from selected Nigerian public hospitals using structured patient questionnaires, healthcare worker questionnaires, communication-skills assessment tools, direct observation checklists, and relevant hospital records. A multistage sampling technique will be employed to select states, public hospitals, clinical departments, healthcare workers, and eligible patients. Healthcare worker communication skills will be assessed using indicators such as active listening, clarity of explanations, use of understandable language, respectful communication, responsiveness to patients' questions, empathy in communication, appropriate non-verbal communication, provision of relevant information, confirmation of patient understanding, explanation of procedures and treatment, communication of risks and instructions, and maintenance of confidentiality during communication. Where feasible, direct observation of healthcare worker-patient interactions or standardized communication assessments will be used to complement self-reported information. Patient satisfaction will be assessed using indicators such as satisfaction with communication, information received, provider responsiveness, respect and courtesy, trust, involvement in healthcare decisions, perceived service quality, willingness to return to the hospital, willingness to recommend the hospital, and overall satisfaction. Additional information will be collected on patients' age, sex, educational level, type of healthcare service received, frequency of hospital visits, and duration of interaction with healthcare workers. Descriptive statistics will be used to summarize healthcare worker communication skills, patient characteristics, and satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of communication skills on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that effective healthcare worker communication skills have a significant positive effect on patient satisfaction in Nigerian public hospitals. Patients who receive clear explanations, respectful treatment, attentive listening, understandable information, and adequate responses to their questions are expected to report higher levels of satisfaction than patients who experience poor communication. Effective communication is expected to improve patients' understanding of their conditions and treatment, strengthen trust in healthcare providers, reduce anxiety, and promote a sense of dignity and involvement in healthcare decisions. Healthcare workers who communicate clearly and respectfully may also create a more supportive environment in which patients feel comfortable expressing their concerns and asking questions. Conversely, poor listening, unclear explanations, dismissive communication, inappropriate language, inadequate information, and failure to respond to patients' concerns may contribute to dissatisfaction and negative perceptions of healthcare quality. However, communication skills alone may not fully determine patient satisfaction because satisfaction is also influenced by waiting time, availability of medicines and equipment, healthcare costs, facility cleanliness, accessibility, clinical outcomes, and the overall hospital environment. Therefore, improving healthcare worker communication should form part of a broader patient-centred healthcare quality strategy. The study is expected to contribute to the literature on healthcare worker communication skills, patient satisfaction, patient-centred care, healthcare quality, health communication, hospital administration, nursing, health-services management, and public health in Nigeria by providing empirical evidence on the relationship between healthcare worker communication and patients' experiences of care. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospital administrators, healthcare professional regulatory bodies, nursing and medical management departments, healthcare quality-improvement teams, healthcare training institutions, development partners, and policymakers regarding strategies for improving healthcare communication. The study will also provide evidence-based recommendations for strengthening communication-skills training, improving active listening and patient counselling, promoting the use of patient-friendly language, strengthening communication during diagnosis and treatment, improving information-sharing practices, encouraging patient participation in healthcare decisions, and incorporating communication quality into routine patient-satisfaction and healthcare-quality assessments in Nigerian public hospitals.
