Abstract

Healthcare provider empathy is an important component of patient-centred healthcare because it involves the ability of healthcare professionals to understand patients' experiences, concerns, emotions, and needs and respond to them with appropriate understanding, respect, and compassion. Empathetic healthcare interactions may improve communication, strengthen trust, reduce patient anxiety, encourage patients to provide relevant health information, and enhance their overall healthcare experience. Patient satisfaction refers to the extent to which patients' expectations and perceptions regarding the healthcare services they receive are met, including satisfaction with provider interaction, communication, responsiveness, quality of care, waiting time, respect, and overall healthcare experience. In Nigerian hospitals, patients may encounter challenges such as long waiting times, high patient volumes, resource constraints, communication difficulties, and stressful healthcare environments, which may affect interactions between healthcare providers and patients. Provider empathy may therefore play an important role in shaping patients' perceptions of the quality and acceptability of healthcare services. Against this background, this study investigates the effect of healthcare provider empathy on patient satisfaction in Nigerian hospitals. The study is anchored on the Patient-Centred Care Theory, the Empathy-Altruism Perspective, and Expectancy-Disconfirmation Theory. Patient-Centred Care Theory emphasizes respect for patients' preferences, values, needs, and experiences and recognizes effective communication and compassionate interaction as essential components of quality healthcare. The Empathy-Altruism Perspective provides a framework for understanding how healthcare providers' ability to recognize and respond to patients' emotional and informational needs may influence healthcare interactions and patient experiences. Expectancy-Disconfirmation Theory explains patient satisfaction as the result of comparing patients' expectations of healthcare services with their actual experiences, including the interpersonal behaviour of healthcare providers. Collectively, these theoretical perspectives provide a suitable framework for explaining how healthcare provider empathy may influence patient satisfaction in Nigerian hospitals. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from patients attending selected Nigerian hospitals using a structured self-administered or interviewer-administered questionnaire incorporating a validated healthcare provider empathy assessment scale and patient satisfaction scale. A multistage sampling technique will be employed to select states, hospitals, departments or service units, healthcare providers, and eligible patients. Healthcare provider empathy will be assessed using indicators such as active listening, understanding of patients' concerns, respectful communication, emotional support, compassion, consideration of patients' perspectives, patience, explanation of medical information, and responsiveness to patients' questions and needs. Patient satisfaction will be assessed using indicators such as satisfaction with provider communication, respect, responsiveness, quality of care, waiting time, involvement in decision-making, trust in healthcare providers, and overall satisfaction. Additional information will be collected on patients' age, sex, educational level, type of healthcare service received, frequency of hospital visits, waiting time, and relevant healthcare characteristics. Descriptive statistics will be used to summarize participants' characteristics, perceived provider empathy, and patient satisfaction levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of healthcare provider empathy on patient satisfaction. Diagnostic tests will also be conducted to assess the reliability of the research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that higher levels of healthcare provider empathy have a significant positive effect on patient satisfaction in Nigerian hospitals. Patients who perceive healthcare providers as understanding, respectful, compassionate, attentive, and responsive to their concerns are expected to report greater satisfaction with the healthcare services received. Empathetic communication is expected to improve patients' understanding of their conditions and treatment, strengthen trust, encourage open communication, and reduce anxiety associated with hospital care. Patients who feel respected and listened to are also expected to demonstrate greater confidence in their healthcare providers and hospitals. Conversely, perceived lack of empathy, dismissive communication, inadequate listening, or insensitive interactions may contribute to dissatisfaction and reduced confidence in healthcare services. However, provider empathy alone may not determine patient satisfaction because patients' experiences may also be influenced by waiting time, healthcare costs, availability of medicines and equipment, clinical outcomes, facility conditions, and accessibility of services. Therefore, improving provider empathy should form part of a broader patient-centred healthcare quality strategy. The study is expected to contribute to the literature on healthcare provider empathy, patient satisfaction, patient-centred care, healthcare quality, provider-patient relationships, hospital services, communication, and public health in Nigeria by providing empirical evidence on the role of interpersonal healthcare interactions in shaping patient experiences. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare institutions, medical and nursing schools, healthcare professional regulatory bodies, hospital administrators, physicians, nurses, pharmacists, other healthcare professionals, quality-improvement teams, and policymakers regarding strategies for improving patient-centred healthcare. The study will also provide evidence-based recommendations for strengthening empathy and communication training, incorporating patient-centred care into healthcare professional education, improving provider-patient communication, establishing patient-experience monitoring systems, and promoting respectful, compassionate, and responsive healthcare delivery in Nigerian hospitals.
