Effect of Health Promotion Campaigns on Breastfeeding Initiation among Nursing Mothers in Nigeria
Abstract
Breastfeeding is an important component of maternal and child health because it provides essential nutrients and protective factors that support infant growth, development, and protection against infections. Early initiation of breastfeeding, particularly initiation within the first hour after birth, is recommended as an important newborn-care practice and is associated with improved breastfeeding outcomes and child survival. However, breastfeeding initiation practices in Nigeria may be affected by inadequate knowledge, cultural beliefs, misconceptions, social influences, inadequate professional support, and limited exposure to appropriate breastfeeding information. Health promotion campaigns provide an avenue for disseminating evidence-based information, addressing misconceptions, promoting healthy maternal behaviours, and encouraging mothers to adopt recommended breastfeeding practices. Health promotion campaigns refer to organized communication and educational activities designed to increase awareness, knowledge, motivation, and adoption of recommended health behaviours through channels such as community outreach, mass media, social media, healthcare facilities, interpersonal communication, and public awareness programmes. Breastfeeding initiation refers to the commencement of breastfeeding after childbirth, particularly whether the mother initiates breastfeeding within the recommended period following delivery. Against this background, this study investigates the effect of health promotion campaigns on breastfeeding initiation among nursing mothers in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Theory of Planned Behavior. The Health Belief Model explains health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action. Social Cognitive Theory emphasizes the interaction between individual knowledge, behavioural factors, social influences, and the environment in shaping health behaviours. The Theory of Planned Behavior explains behavioural intention and action through attitudes, subjective norms, and perceived behavioural control. Collectively, these theoretical perspectives provide a suitable framework for explaining how health promotion campaigns may influence breastfeeding initiation among nursing mothers in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from nursing mothers attending selected healthcare facilities and community health programmes across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, communities, and eligible nursing mothers. Health promotion campaigns will be assessed using indicators such as exposure to breastfeeding messages, frequency of campaign exposure, campaign content, use of mass media, social media communication, community outreach, healthcare-facility education, interpersonal counselling, involvement of community health workers, and exposure to breastfeeding demonstrations or educational materials. Breastfeeding initiation will be assessed using indicators such as whether breastfeeding was initiated after delivery, time from delivery to first breastfeeding, initiation within the recommended first hour, and whether colostrum was given to the newborn. Structured questionnaires will be used for data collection, while maternal and child health records may be reviewed where available to validate reported information. Descriptive statistics will be used to summarize participants' characteristics, exposure to health promotion campaigns, and breastfeeding initiation patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of health promotion campaigns on breastfeeding initiation. Diagnostic tests will also be conducted to assess the reliability of the research instrument and the robustness of the findings. The study is expected to find that exposure to effective health promotion campaigns has a significant positive effect on breastfeeding initiation among nursing mothers in Nigeria. Mothers who receive clear, consistent, and evidence-based breastfeeding information are expected to be more likely to initiate breastfeeding within the recommended period after delivery. Health promotion campaigns may improve maternal knowledge regarding the benefits of early breastfeeding, the importance of colostrum, appropriate breastfeeding techniques, and the potential risks associated with delayed initiation. Campaigns delivered through healthcare facilities, community health workers, mass media, social media, and community-based platforms may increase access to breastfeeding information and reinforce recommended practices. Conversely, limited exposure to appropriate health promotion messages may contribute to inadequate knowledge, misconceptions, and delayed breastfeeding initiation. However, health promotion campaigns alone may not fully determine breastfeeding initiation because delivery practices, maternal health conditions, healthcare-worker support, cultural beliefs, family influence, workplace and social support, and facility-level breastfeeding practices may also influence maternal decisions and behaviours. The study is expected to contribute to the literature on health promotion, breastfeeding initiation, maternal and child health, infant nutrition, community health, health education, and public health in Nigeria by providing empirical evidence on the role of health promotion campaigns in encouraging recommended breastfeeding practices. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, primary healthcare authorities, hospitals, maternity facilities, healthcare workers, community health workers, nutritionists, maternal and child health programmes, development partners, and policymakers regarding strategies for improving breastfeeding practices. The study will also provide evidence-based recommendations for strengthening breastfeeding promotion campaigns, improving maternal health education, increasing community-based breastfeeding education, engaging healthcare workers and community leaders, expanding the use of mass and digital media, addressing cultural misconceptions, and integrating breastfeeding promotion into antenatal, maternity, postnatal, and community health services across Nigeria.
