Effect of Growth Monitoring Services on Malnutrition Detection among Children Under Five in Nigeria
Abstract
Malnutrition remains an important public health concern among children under five years in Nigeria and can adversely affect physical growth, cognitive development, immunity, and overall child survival. Early identification of malnutrition is essential for providing timely nutritional counselling, treatment, referral, and follow-up before nutritional problems become severe. Growth monitoring services provide an opportunity for the regular assessment of children's physical growth and nutritional status through measurements such as weight, height or length, mid-upper arm circumference, and other appropriate anthropometric indicators. Growth monitoring refers to the routine measurement and interpretation of a child's growth over time to identify deviations from expected growth patterns and possible nutritional problems. Malnutrition detection refers to the identification of children who are underweight, stunted, wasted, or otherwise at risk of nutritional deficiencies using established anthropometric and clinical criteria. In Nigeria, inadequate access to child health services, irregular attendance at growth-monitoring sessions, limited availability of trained health workers, inadequate equipment, poor documentation, and low caregiver awareness may reduce opportunities for early detection of malnutrition. Strengthening growth monitoring services may improve the identification of children with nutritional problems and facilitate timely intervention. Against this background, this study investigates the effect of growth monitoring services on malnutrition detection among children under five in Nigeria. The study will be anchored on the Health Systems Framework, Donabedian's Structure-Process-Outcome Model, and the Health Belief Model. The Health Systems Framework recognizes service delivery, health workforce, health information, medical technologies, and access to essential health services as important components of an effective healthcare system. Donabedian's Model explains how healthcare structures and processes influence health outcomes, providing a basis for understanding how the availability and quality of growth monitoring services may affect malnutrition detection. The Health Belief Model explains caregivers' utilization of preventive child health services through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action. Collectively, these theoretical perspectives provide a suitable framework for explaining how growth monitoring services may influence malnutrition detection among children under five in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from caregivers of children aged 0–59 months attending selected primary healthcare facilities and community-based child health programmes across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, and eligible caregiver-child pairs. Growth monitoring services will be assessed using indicators such as availability of growth monitoring sessions, frequency of attendance, availability of functional weighing scales and length/height-measuring equipment, availability of trained healthcare workers, regularity of anthropometric measurements, maintenance of child growth charts, quality of growth assessment, caregiver counselling, documentation, and referral procedures. Malnutrition detection will be assessed using anthropometric indicators such as weight-for-age, height-for-age, weight-for-height or body mass index-for-age where appropriate, and mid-upper arm circumference, based on recognized child growth standards. The study will assess the proportion of children with previously undetected undernutrition identified through growth monitoring and the timeliness of referral or intervention following abnormal findings. Facility records and child health cards will be reviewed where available to validate reported information. Descriptive statistics will be used to summarize participants' characteristics, utilization of growth monitoring services, and malnutrition detection patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of growth monitoring services on malnutrition detection. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that effective growth monitoring services have a significant positive effect on malnutrition detection among children under five in Nigeria. Children who regularly receive growth monitoring are expected to have a greater likelihood of having previously undetected nutritional problems identified than children with limited access to such services. Regular anthropometric assessment may facilitate the early identification of wasting, stunting, underweight, and other growth abnormalities and provide opportunities for timely nutritional counselling, treatment, referral, and follow-up. Growth charts may also enable healthcare workers to identify changes in growth patterns that may require further assessment. Conversely, inadequate or irregular growth monitoring may result in missed opportunities to identify malnutrition at an early stage and may allow nutritional problems to progress without appropriate intervention. However, growth monitoring alone may not fully determine malnutrition detection because availability of trained personnel, functional equipment, caregiver attendance, quality of record-keeping, healthcare accessibility, and the availability of referral and treatment services may also influence detection. The study is expected to contribute to the literature on growth monitoring, malnutrition detection, child nutrition, maternal and child health, community health, primary healthcare, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare development agencies, healthcare facilities, community health workers, nutritionists, development partners, and policymakers regarding strategies for improving early identification of childhood malnutrition. The study will also provide evidence-based recommendations for strengthening growth monitoring services, ensuring availability of functional anthropometric equipment, training healthcare workers, improving caregiver education, strengthening documentation and growth-chart utilization, improving referral systems, and integrating routine growth monitoring into comprehensive child health and nutrition services across Nigeria.
