Effect of Fertility Awareness Education on Fertility Knowledge among Women of Reproductive Age in Nigeria

 

Abstract

Fertility knowledge is an important component of reproductive health because it enables women to understand factors that influence reproductive potential, make informed decisions about pregnancy planning, recognize fertility-related concerns, and seek appropriate reproductive healthcare. Women of reproductive age may have inadequate knowledge of ovulation, the fertile window, menstrual-cycle patterns, age-related changes in fertility, reproductive health conditions, sexually transmitted infections, lifestyle factors, and appropriate timing for fertility assessment. In Nigeria, gaps in fertility knowledge may be influenced by limited access to reliable reproductive health information, cultural beliefs, misconceptions, stigma surrounding fertility problems, and inadequate reproductive health education. Fertility awareness education provides an opportunity to improve women's understanding of fertility and support informed reproductive health decisions. Against this background, this study investigates the effect of fertility awareness education on fertility knowledge among women of reproductive age in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice Model. The Health Belief Model explains how women's perceptions of susceptibility to fertility-related health problems, perceived severity, perceived benefits of fertility awareness, perceived barriers, and cues to action may influence their acquisition of fertility knowledge. Social Cognitive Theory emphasizes knowledge acquisition, self-efficacy, observational learning, social interaction, and environmental influences in the development and application of reproductive health knowledge. The Knowledge-Attitude-Practice Model explains how exposure to appropriate fertility information may improve knowledge and influence subsequent reproductive health decisions. Collectively, these theoretical perspectives provide a suitable framework for explaining how fertility awareness education may influence fertility knowledge among women of reproductive age in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise women of reproductive age attending selected healthcare facilities, reproductive health clinics, fertility clinics, community health programmes, workplaces, and other relevant settings across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, healthcare facilities, organizations, and eligible women. Fertility awareness education will be assessed based on exposure to structured educational sessions covering ovulation, the menstrual cycle, fertile periods, age-related fertility changes, factors affecting fertility, sexually transmitted infections, lifestyle factors, pregnancy planning, fertility preservation where appropriate, and indications for professional fertility assessment. Fertility knowledge will be assessed using participants' ability to correctly identify fertility-related concepts, including ovulation, fertile periods, menstrual-cycle patterns, age-related fertility changes, reproductive health risk factors, effects of sexually transmitted infections, lifestyle-related fertility factors, and appropriate circumstances for seeking professional fertility assessment. Data will be collected using structured questionnaires and validated or appropriately developed fertility knowledge assessment instruments. Descriptive statistics will be used to summarize participants' characteristics and fertility knowledge levels, while inferential techniques such as chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis will be used to determine the effect of fertility awareness education on fertility knowledge. Where a quasi-experimental design is used, pre-intervention and post-intervention fertility knowledge scores will be compared to determine changes associated with the educational intervention. The study is expected to find that fertility awareness education has a significant positive effect on fertility knowledge among women of reproductive age in Nigeria. Women exposed to structured and evidence-based fertility awareness education are expected to demonstrate higher levels of knowledge concerning ovulation, fertile periods, menstrual-cycle patterns, age-related fertility changes, reproductive health conditions, sexually transmitted infections, lifestyle factors, and appropriate fertility-related healthcare-seeking than women with limited exposure to fertility education. Improved fertility knowledge may enable women to make more informed decisions regarding pregnancy planning and recognize circumstances that may require professional reproductive health assessment. However, cultural beliefs, misinformation from peers and social media, low health literacy, stigma surrounding fertility problems, limited access to reliable reproductive health information, and socioeconomic barriers may reduce the effectiveness of fertility awareness education. The study therefore expects accessible, culturally appropriate, evidence-based, and regularly reinforced fertility awareness education to contribute significantly to improved fertility knowledge among women of reproductive age in Nigeria. The study is expected to contribute to the literature on fertility awareness education, fertility knowledge, reproductive health education, women's reproductive health, pregnancy planning, fertility health, infertility prevention, health literacy, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, healthcare facilities, reproductive health and fertility clinics, healthcare providers, gynaecologists, nurses, midwives, community health workers, women's health organizations, development partners, and policymakers regarding strategies for improving fertility knowledge among women. The study will also provide evidence-based recommendations for integrating fertility awareness education into appropriate reproductive and primary healthcare services, training healthcare workers and peer educators, developing culturally appropriate fertility education materials, strengthening community-based reproductive health education, improving access to reliable fertility information, and promoting informed reproductive health decision-making among women of reproductive age across Nigeria.
