Effect of Electronic Medical Records on Patient Record Retrieval in Nigerian Hospitals
Abstract
Efficient management and retrieval of patient records are essential components of effective healthcare delivery because healthcare professionals require timely access to accurate patient information for diagnosis, treatment, continuity of care, clinical decision-making, and administrative processes. In many Nigerian hospitals, reliance on paper-based medical records may result in difficulties such as misplaced files, incomplete documentation, delayed retrieval, physical storage limitations, duplication of records, and increased workload for health information personnel. Electronic Medical Records (EMRs) provide a digital approach to storing, organizing, managing, and retrieving patient health information and may improve the speed, accessibility, accuracy, and efficiency of medical record management. Electronic Medical Records refer to computerized systems used to create, store, manage, and retrieve patients' health information within healthcare facilities. Patient record retrieval refers to the process of locating, accessing, and making patients' medical records available to authorized healthcare personnel when required. In Nigeria, increasing adoption of digital health technologies provides opportunities to improve health information management, although challenges such as inadequate infrastructure, unreliable electricity supply, limited technical skills, poor system maintenance, inadequate funding, and resistance to technological change may affect EMR implementation. Against this background, this study investigates the effect of Electronic Medical Records on patient record retrieval in Nigerian hospitals. The study will be anchored on the Technology Acceptance Model, the DeLone and McLean Information Systems Success Model, and the Health Systems Framework. The Technology Acceptance Model explains technology adoption through perceived usefulness and perceived ease of use and provides a framework for understanding healthcare workers' acceptance and use of EMR systems. The DeLone and McLean Information Systems Success Model emphasizes system quality, information quality, service quality, system use, user satisfaction, and net benefits in determining information-system effectiveness. The Health Systems Framework recognizes health information systems, infrastructure, human resources, and service delivery as essential components of effective healthcare systems. Collectively, these theoretical perspectives provide a suitable framework for explaining how EMR implementation may influence patient record retrieval in Nigerian hospitals. The study will adopt a quantitative cross-sectional research design. Data will be collected from selected public and private hospitals using healthcare workers and health information management personnel who regularly use or manage patient records. A multistage sampling technique will be used to select states, local government areas, hospitals, departments, and eligible participants. Electronic Medical Record implementation will be assessed using indicators such as availability of functional EMR systems, extent of electronic record use, digital registration, electronic storage, computerized indexing, electronic search functions, system accessibility, user training, system maintenance, and availability of technical support. Patient record retrieval will be assessed using indicators such as retrieval time, ease of locating records, completeness of retrieved records, frequency of misplaced records, record accessibility, retrieval errors, duplication of records, and availability of records when required for patient care. Structured questionnaires, system observation checklists, and health information department records will be used for data collection where available. Descriptive statistics will be used to summarize participants' characteristics, EMR implementation, and patient record retrieval patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of Electronic Medical Records on patient record retrieval. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that effective implementation of Electronic Medical Records has a significant positive effect on patient record retrieval in Nigerian hospitals. Hospitals with functional EMR systems are expected to retrieve patient records more quickly and efficiently than hospitals that rely predominantly on paper-based systems. Electronic search and indexing functions may reduce the time required to locate patient information and reduce the risk of misplaced or physically inaccessible records. Digital storage may also improve record availability across authorized departments and support continuity of care. Furthermore, electronic records may reduce duplication and improve the organization of patient information. Conversely, poorly implemented EMR systems, inadequate staff training, unreliable electricity supply, system downtime, insufficient technical support, and poor data management practices may limit the benefits of electronic records and create additional retrieval challenges. However, EMR implementation alone may not fully determine record retrieval efficiency because staff competence, hospital infrastructure, system quality, data-entry practices, network connectivity, technical support, and organizational procedures may also influence retrieval performance. The study is expected to contribute to the literature on Electronic Medical Records, patient record retrieval, health information management, digital health, health informatics, hospital administration, healthcare efficiency, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, hospital management boards, health information management departments, healthcare administrators, health informatics professionals, healthcare workers, and policymakers regarding strategies for improving digital health information management. The study will also provide evidence-based recommendations for expanding functional EMR systems, improving hospital digital infrastructure, strengthening staff training, ensuring reliable electricity and network connectivity, improving technical support, strengthening data management procedures, and establishing effective EMR maintenance and monitoring systems to improve patient record retrieval in Nigerian hospitals.
