Effect of Digital Health Literacy on Telemedicine Utilization among Adults in Nigeria

Abstract

Digital health literacy is increasingly important as healthcare systems adopt digital technologies to improve access to healthcare services. Digital health literacy refers to the ability of individuals to locate, understand, evaluate, communicate, and appropriately use health information and digital health technologies to make informed healthcare decisions. Telemedicine utilization refers to the use of telecommunications and digital technologies to obtain healthcare services remotely, including virtual consultations, telephone consultations, video consultations, remote follow-up, digital appointment systems, and other technology-enabled healthcare services. In Nigeria, telemedicine has the potential to improve access to healthcare, particularly for individuals living in underserved communities and those who face geographical, financial, or transportation barriers to conventional healthcare facilities. However, limited digital health literacy, inadequate knowledge of available telemedicine services, difficulties navigating digital platforms, concerns about privacy and security, limited digital skills, poor internet connectivity, and unequal access to digital devices may constrain telemedicine utilization. Individuals with adequate digital health literacy may be better able to identify appropriate telemedicine services, navigate digital healthcare platforms, assess online health information, communicate with healthcare providers, and use remote healthcare services effectively. Against this background, this study investigates the effect of digital health literacy on telemedicine utilization among adults in Nigeria. The study is anchored on the Technology Acceptance Model, the Digital Divide Theory, and the Health Belief Model. The Technology Acceptance Model explains technology utilization through users' perceptions of usefulness and ease of use, which may be influenced by their ability to understand and navigate digital health technologies. The Digital Divide Theory emphasizes inequalities in access to digital technologies, digital skills, and the ability to benefit from digital services. The Health Belief Model provides a framework for understanding how individuals' knowledge, perceptions of health needs, perceived benefits, and perceived barriers may influence their use of available healthcare services. Collectively, these theoretical perspectives provide a suitable framework for explaining how digital health literacy may influence telemedicine utilization among adults in Nigeria. The study will adopt a quantitative cross-sectional research design. Primary data will be collected from adults in selected Nigerian communities using structured digital health literacy questionnaires and telemedicine utilization assessment instruments. A multistage sampling technique will be employed to select states, local government areas, communities, households, and eligible adult participants. Digital health literacy will be assessed using indicators such as ability to search for online health information, ability to identify credible health information sources, ability to understand digital health information, ability to evaluate the reliability of online health content, ability to navigate telemedicine platforms, ability to communicate with healthcare providers through digital platforms, ability to protect personal health information online, and confidence in using digital health technologies. Telemedicine utilization will be assessed using indicators such as previous use of telephone consultations, video consultations, online medical consultations, remote follow-up services, electronic appointment systems, digital prescription services, frequency of telemedicine use, regularity of use, and willingness to use telemedicine when appropriate. Information will also be collected on participants' age, sex, educational level, employment status, residence, internet access, smartphone ownership, and previous experience with digital technologies. Descriptive statistics will be used to summarize digital health literacy levels and patterns of telemedicine utilization. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of digital health literacy on telemedicine utilization. Diagnostic tests will also be conducted to assess the reliability of research instruments, model assumptions, goodness of fit, multicollinearity, and robustness of the findings. The study is expected to find that higher levels of digital health literacy have a significant positive effect on telemedicine utilization among adults in Nigeria. Adults with stronger digital health literacy are expected to demonstrate greater ability and confidence in locating appropriate telemedicine services, registering on digital healthcare platforms, communicating with healthcare providers remotely, evaluating digital health information, and using telemedicine services when appropriate. Higher digital health literacy may also reduce uncertainty and perceived difficulty associated with digital healthcare platforms, thereby encouraging greater utilization. Conversely, low digital health literacy may limit adults' ability to identify appropriate digital healthcare services, navigate telemedicine platforms, distinguish credible health information from unreliable online content, and communicate effectively with healthcare providers through digital technologies. However, digital health literacy alone may not fully determine telemedicine utilization because use is also influenced by internet connectivity, smartphone ownership, affordability of data services, availability of telemedicine providers, privacy concerns, trust in digital healthcare, healthcare needs, and the availability of electricity and digital infrastructure. Therefore, improving digital health literacy should be accompanied by improvements in digital healthcare infrastructure and accessibility. The study is expected to contribute to the literature on digital health literacy, telemedicine utilization, digital healthcare, health informatics, healthcare access, health communication, and public health in Nigeria by providing empirical evidence on the role of digital competencies in the adoption of remote healthcare services. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, Nigeria Digital Health Strategy stakeholders, state ministries of health, healthcare providers, telemedicine service providers, digital-health organizations, telecommunications stakeholders, development partners, and policymakers regarding strategies for improving digital healthcare utilization. The study will also provide evidence-based recommendations for strengthening digital health literacy programmes, incorporating digital-health education into community health programmes, improving user-friendly telemedicine platforms, expanding digital-health awareness campaigns, strengthening digital privacy education, improving access to digital devices and internet services, and integrating telemedicine literacy into broader healthcare-access initiatives in Nigeria.
