Effect of Community-Based Rehabilitation Services on Functional Independence among Persons with Disabilities in Nigeria

 

Abstract

Persons with disabilities may experience limitations in mobility, communication, self-care, participation in education and employment, and other activities of daily living. Access to appropriate rehabilitation services is therefore important for improving functional ability, independence, social participation, and quality of life. Community-based rehabilitation services provide rehabilitation and support within or close to the communities where persons with disabilities live and may include physical rehabilitation, occupational therapy, assistive-device support, communication services, functional training, health education, referral, and social support. Functional independence refers to the extent to which a person with a disability can perform essential activities of daily living and participate in routine activities with limited assistance from others. In Nigeria, persons with disabilities may experience barriers to rehabilitation services, including limited availability of rehabilitation professionals, inadequate assistive devices, financial constraints, inaccessible facilities, geographical distance, limited community-based programmes, and insufficient disability-related health services. Strengthening community-based rehabilitation may bring rehabilitation services closer to persons with disabilities and improve their ability to perform daily activities independently. Against this background, this study investigates the effect of community-based rehabilitation services on functional independence among persons with disabilities in Nigeria. The study will be anchored on the Social Model of Disability, the International Classification of Functioning, Disability and Health (ICF) Framework, and the Health Systems Framework. The Social Model of Disability emphasizes that disability is influenced not only by an individual's impairment but also by environmental, social, and institutional barriers to participation. The ICF Framework conceptualizes functioning and disability in relation to body functions and structures, activities, participation, environmental factors, and personal factors and provides a suitable framework for assessing functional independence. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical technologies, financing, and access to healthcare as important components of effective rehabilitation systems. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based rehabilitation services may influence functional independence among persons with disabilities in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from persons with disabilities who receive or have access to community-based rehabilitation services across selected locations in Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, rehabilitation programmes, and eligible participants. Community-based rehabilitation services will be assessed using indicators such as availability of rehabilitation services within the community, frequency of rehabilitation sessions, accessibility of rehabilitation personnel, physical rehabilitation, occupational rehabilitation, speech and communication support, assistive-device provision, functional skills training, health education, referral services, follow-up support, affordability, and geographical accessibility. Functional independence will be assessed using appropriate standardized measures of activities of daily living and instrumental activities of daily living, covering areas such as mobility, personal hygiene, dressing, feeding, toileting, communication, household activities, and participation in routine activities. Structured questionnaires, service records, and appropriate functional assessment tools will be used for data collection. Descriptive statistics will be used to summarize participants' characteristics, access to community-based rehabilitation services, and functional independence levels. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of community-based rehabilitation services on functional independence. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that access to effective community-based rehabilitation services has a significant positive effect on functional independence among persons with disabilities in Nigeria. Persons who regularly receive appropriate rehabilitation services are expected to demonstrate greater ability to perform activities of daily living and participate in community activities than those with limited access to rehabilitation services. Physical and occupational rehabilitation may improve mobility, strength, coordination, self-care skills, and ability to perform routine activities. Access to appropriate assistive devices and training may further enhance independence and reduce dependence on caregivers. Community-based delivery may also reduce geographical and financial barriers associated with accessing distant rehabilitation facilities. Conversely, inadequate rehabilitation coverage, insufficient rehabilitation professionals, limited assistive devices, irregular service provision, and high service costs may restrict improvements in functional independence. However, community-based rehabilitation services alone may not fully determine functional independence because the type and severity of disability, age, socioeconomic circumstances, family support, accessibility of the physical environment, availability of assistive technologies, and social inclusion opportunities may also influence functional ability. The study is expected to contribute to the literature on community-based rehabilitation, disability health, functional independence, rehabilitation services, health equity, community health, social care, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, National Commission for Persons with Disabilities, state ministries of health, rehabilitation centres, healthcare providers, disability organizations, community-based organizations, development partners, and policymakers regarding strategies for improving rehabilitation services. The study will also provide evidence-based recommendations for expanding community-based rehabilitation programmes, increasing the availability of rehabilitation professionals, improving access to assistive devices, strengthening community rehabilitation networks, reducing financial and geographical barriers, improving referral and follow-up systems, and integrating disability-inclusive rehabilitation services into primary healthcare and community health programmes across Nigeria.
