Effect of Community-Based Eye Screening on Refractive Error Detection among School Children in Nigeria
Abstract
Refractive errors remain an important cause of visual impairment among school children in Nigeria and may include myopia, hypermetropia, and astigmatism. Undetected refractive errors can negatively affect children's classroom participation, reading ability, academic performance, social interaction, and overall development. Many school children may experience difficulties with vision without receiving appropriate eye assessment because of limited awareness among parents and teachers, inadequate access to eye-care services, financial constraints, geographical barriers, and shortages of trained eye-care personnel. Early detection of refractive errors is important because timely correction through appropriate spectacles or other suitable interventions can improve visual function and reduce the educational and social consequences of uncorrected visual impairment. Community-based eye screening provides an opportunity to bring basic eye assessment closer to children through schools, community health centres, outreach programmes, mobile eye clinics, and other community platforms. However, inadequate screening coverage, limited availability of eye-care professionals and equipment, poor referral systems, low parental awareness, and limited access to corrective spectacles may reduce the effectiveness of community-based eye screening programmes in Nigeria. Against this background, this study investigates the effect of community-based eye screening on refractive error detection among school children in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and the Health Systems Framework. The Health Belief Model explains how parents', teachers', and children's perceptions of susceptibility to visual problems, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in eye screening. Community Health Theory emphasizes the delivery of accessible health services through community structures and the involvement of communities in disease prevention, health promotion, and early detection activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective eye-care and screening programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based eye screening may influence refractive error detection among school children in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise school children enrolled in selected primary and secondary schools across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, schools, classes, and eligible school children. Community-based eye screening will be measured using indicators such as availability of screening services, screening coverage, frequency of screening activities, accessibility of screening locations, number of children screened, visual acuity assessment, availability of trained eye-care personnel, availability of screening equipment, school-based outreach activities, and referral arrangements. Refractive error detection will be assessed using indicators such as abnormal visual acuity results, suspected refractive errors, identified cases of myopia, hypermetropia, and astigmatism, refractive error measurements following appropriate examination, number of children referred for comprehensive eye assessment, confirmed cases, and provision or recommendation of corrective spectacles where appropriate. Data will be collected using structured questionnaires, eye screening registers, visual acuity records, optometric assessment records, referral registers, school health records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, visual assessment findings, and patterns of refractive error detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of community-based eye screening on refractive error detection. Where appropriate, refractive error detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based eye screening has a significant positive effect on refractive error detection among school children in Nigeria. School children who participate in community-based eye screening are expected to have a higher likelihood of having previously undetected refractive errors identified than children without access to regular eye screening. Routine visual acuity assessment and appropriate follow-up examination may facilitate early identification of myopia, hypermetropia, astigmatism, and other visual abnormalities. Early detection may allow children to receive appropriate corrective spectacles, referral for comprehensive eye examination, and necessary follow-up care. Community-based screening may also reduce geographical and financial barriers by bringing basic eye assessment services closer to schools and communities. Improved detection may contribute to better classroom participation, reading ability, academic performance, and overall well-being. Conversely, inadequate screening coverage, shortages of trained eye-care personnel, limited screening equipment, poor referral systems, low parental awareness, and limited access to corrective spectacles may reduce the effectiveness of community-based eye screening. The study therefore expects accessible and well-organized community-based eye screening to contribute significantly to improved detection of refractive errors among school children in Nigeria. The study is expected to contribute to the literature on community-based eye screening, refractive error detection, school eye health, visual impairment, child health, preventive eye care, school health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, National Primary Health Care Development Agency, state ministries of health and education, primary healthcare centres, schools, optometrists, ophthalmologists, eye-care organizations, community health workers, development partners, and policymakers regarding strategies for improving children's eye health. The study will also provide evidence-based recommendations for expanding community-based eye screening programmes, integrating school eye health services into primary healthcare and education systems, increasing the availability of trained eye-care personnel and screening equipment, improving referral and follow-up systems, increasing parental and teacher awareness of childhood vision problems, improving access to affordable corrective spectacles, and strengthening preventive eye-care services for school children across Nigeria.
