Abstract

Cholera remains an important public health challenge in Nigeria, particularly in communities affected by inadequate access to safe drinking water, poor sanitation, overcrowding, flooding, and limited access to timely healthcare services. Effective control of cholera outbreaks requires rapid detection, prompt investigation, timely case management, effective risk communication, water and sanitation interventions, community mobilization, and coordinated public health action. Cholera Rapid Response Teams (RRTs) are multidisciplinary teams established or activated to investigate suspected outbreaks and coordinate immediate response activities. Their responsibilities may include outbreak investigation, case identification, surveillance, laboratory coordination, contact and community assessment, risk communication, infection prevention and control, water and sanitation interventions, case referral, and coordination with relevant government and community stakeholders. Cholera outbreak control refers to the ability of public health authorities and response systems to detect, contain, and reduce the transmission and impact of cholera outbreaks within affected communities. In Nigeria, challenges such as inadequate response resources, limited laboratory capacity, poor coordination, delayed reporting, transportation difficulties, shortages of essential supplies, and inadequate water and sanitation infrastructure may hinder effective outbreak control. Strengthening Cholera Rapid Response Teams may improve the timeliness and coordination of outbreak response activities and contribute to more effective containment of cholera outbreaks. Against this background, this study investigates the effect of Cholera Rapid Response Teams on cholera outbreak control in Nigeria. The study will be anchored on the Health Systems Framework, the Incident Command System, and the Disease Outbreak Response Framework. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical technologies, financing, and governance as essential components of effective public health response systems. The Incident Command System emphasizes coordinated leadership, clearly defined responsibilities, communication, resource management, and inter-agency coordination during emergencies. The Disease Outbreak Response Framework emphasizes surveillance, early detection, investigation, laboratory confirmation, case management, risk communication, community engagement, and implementation of appropriate control measures. Collectively, these theoretical perspectives provide a suitable framework for explaining how Cholera Rapid Response Teams may influence outbreak control in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from public health officers, epidemiologists, laboratory personnel, environmental health officers, healthcare workers, and other professionals involved in cholera outbreak response in selected states and local government areas in Nigeria. A multistage sampling technique will be used to select states, local government areas, public health institutions, healthcare facilities, and eligible respondents. Cholera Rapid Response Team effectiveness will be assessed using indicators such as availability of trained team members, response activation time, team readiness, availability of response equipment and supplies, transportation, communication systems, outbreak investigation capacity, laboratory coordination, case identification, risk communication, community engagement, environmental health response, referral coordination, and inter-agency collaboration. Cholera outbreak control will be assessed using indicators such as time taken to contain outbreaks, reduction in new cases, reduction in outbreak duration, completeness of outbreak investigation, timeliness of response interventions, case fatality reduction, effectiveness of water and sanitation interventions, and successful implementation of outbreak control measures. Structured questionnaires, outbreak investigation reports, surveillance records, response-team reports, laboratory records, and relevant public health documents will be used for data collection where available. Descriptive statistics will be used to summarize respondents' characteristics, RRT readiness, and outbreak control patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of Cholera Rapid Response Teams on cholera outbreak control. Diagnostic tests will also be conducted to assess the reliability of research instruments and the robustness of the findings. The study is expected to find that effective Cholera Rapid Response Teams have a significant positive effect on cholera outbreak control in Nigeria. Public health systems with well-trained, adequately equipped, and rapidly deployable response teams are expected to demonstrate more timely outbreak investigation and implementation of control measures than systems with limited response capacity. Rapid deployment may facilitate early case identification, laboratory investigation, risk assessment, and initiation of appropriate interventions. Effective coordination among epidemiologists, laboratory personnel, clinicians, environmental health officers, water and sanitation authorities, and community stakeholders may further improve outbreak response. Risk communication and community engagement activities may increase awareness of preventive measures and encourage early reporting and treatment of suspected cases. Conversely, inadequate staffing, insufficient training, poor transportation, shortages of laboratory supplies, weak communication systems, limited funding, and poor coordination may delay outbreak response and reduce the effectiveness of control measures. However, Cholera Rapid Response Teams alone may not fully determine outbreak control because access to safe water, sanitation infrastructure, healthcare capacity, environmental conditions, community behaviour, surveillance systems, and availability of essential treatment supplies may also influence outbreak outcomes. The study is expected to contribute to the literature on cholera outbreak response, rapid response teams, infectious disease control, outbreak preparedness, epidemiology, emergency public health, disease surveillance, environmental health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Nigeria Centre for Disease Control and Prevention, National Primary Health Care Development Agency, state ministries of health, state epidemiology units, local government health authorities, emergency response agencies, healthcare facilities, development partners, and policymakers regarding strategies for strengthening cholera outbreak response. The study will also provide evidence-based recommendations for improving RRT training and readiness, reducing response activation time, strengthening laboratory and surveillance capacity, ensuring adequate transportation and response supplies, improving inter-agency coordination, strengthening risk communication and community engagement, and integrating rapid response teams with broader water, sanitation, hygiene, surveillance, and outbreak preparedness programmes across Nigeria.
