Effect of Blood Donation Education on Voluntary Blood Donation Practices among University Students in Nigeria

Abstract

Blood donation is an essential public health service because safe and adequate blood supplies are required for the management of severe anaemia, obstetric emergencies, trauma, surgical procedures, cancer treatment, and other life-threatening medical conditions. Voluntary non-remunerated blood donation is an important component of a sustainable blood supply because it can help reduce dependence on replacement and paid donations while supporting the availability of safer blood products. University students represent an important potential donor population because many are within the eligible age range and may be able to participate in regular voluntary blood donation programmes. Blood donation education provides students with information about the importance of blood donation, eligibility requirements, donation procedures, safety, common misconceptions, possible temporary reactions, post-donation care, and the importance of regular voluntary donation. Voluntary blood donation practices refer to behaviours involving the voluntary donation of blood without financial or material inducement, willingness to donate when eligible, participation in organized blood donation drives, and intention to donate regularly in the future. In Nigeria, inadequate knowledge, fear of needles, misconceptions about blood donation, concerns about weakness or health effects, lack of awareness of donation opportunities, and limited access to organized blood donation services may discourage voluntary blood donation among university students. Effective blood donation education may improve students' knowledge, reduce misconceptions and fears, and encourage voluntary participation in blood donation. Against this background, this study investigates the effect of blood donation education on voluntary blood donation practices among university students in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains health-related behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding students' decisions to participate in voluntary blood donation. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping blood donation behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from blood donation education may influence voluntary blood donation practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how blood donation education may influence voluntary blood donation practices among university students in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from undergraduate students in selected universities in Nigeria using a structured anonymous self-administered questionnaire and, where feasible, verification of blood donation through appropriate blood donation records with participants' consent and confidentiality safeguards. A multistage sampling technique will be employed to select states, universities, faculties or departments, academic levels, and eligible students. Where feasible, selected student groups will be assigned to intervention and comparison groups. Students in the intervention group will receive structured blood donation education covering the importance of voluntary blood donation, eligibility requirements, donation procedures, donor safety, common misconceptions, possible temporary reactions, post-donation care, blood safety, and opportunities for voluntary blood donation. Voluntary blood donation practices will be assessed using indicators such as previous voluntary blood donation, participation in organized blood donation drives, willingness to donate when eligible, intention to donate regularly, and actual participation in subsequent blood donation opportunities. Descriptive statistics will be used to summarize students' sociodemographic characteristics, blood donation knowledge, exposure to education, and voluntary blood donation practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and logistic or multiple regression analysis, will be used to determine the effect of blood donation education on voluntary blood donation practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that blood donation education has a significant positive effect on voluntary blood donation practices among university students in Nigeria. Students exposed to structured blood donation education are expected to demonstrate improved knowledge of blood donation and greater willingness and participation in voluntary blood donation. Education may reduce misconceptions about blood donation, improve understanding of donor eligibility and safety, reduce fear and anxiety associated with the donation process, and increase awareness of opportunities for voluntary donation. However, education alone may not guarantee sustained voluntary blood donation because students may continue to face barriers such as limited access to organized donation centres, inconvenient donation schedules, fear of needles, lack of reminders, misconceptions, and inadequate opportunities to donate. Consequently, blood donation education is expected to be most effective when supported by accessible blood donation services, regular university-based blood drives, trained blood donation personnel, convenient donation schedules, appropriate donor recognition, and effective follow-up and reminder systems. The study is expected to contribute to the literature on voluntary blood donation, health education, blood safety, university student health, health promotion, and public health in Nigeria by providing empirical evidence on the effect of blood donation education on voluntary blood donation practices among university students. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Commission, National Universities Commission, university administrations, university health centres, blood transfusion services, healthcare professionals, public health practitioners, student organizations, development partners, and policymakers regarding strategies for strengthening voluntary blood donation. The study will also provide evidence-based recommendations for integrating blood donation education into university health programmes, increasing the frequency and accessibility of campus blood donation drives, improving donor education and counselling, addressing misconceptions and fears, strengthening donor recruitment and reminder systems, and promoting regular voluntary blood donation among university students across Nigeria.
