Effect of Air Pollution Awareness on Respiratory Disease Prevention Practices among Urban Residents in Nigeria

Abstract

Air pollution is an important environmental and public health concern in Nigeria, particularly in urban communities where residents may be exposed to pollutants from traffic emissions, industrial activities, household energy use, construction activities, open waste burning, and other sources. Prolonged exposure to polluted air may increase the risk of respiratory conditions such as asthma, chronic respiratory diseases, respiratory infections, and other adverse health outcomes. Air pollution awareness provides residents with information about major sources of air pollution, common pollutants, health effects of exposure, vulnerable populations, personal and household exposure-reduction measures, and the importance of seeking appropriate healthcare when respiratory symptoms occur. Respiratory disease prevention practices refer to behaviours adopted by individuals to reduce exposure to air pollutants and prevent or reduce respiratory health problems, including reducing exposure to polluted environments, avoiding smoke and open burning where possible, improving household ventilation, using appropriate protective measures in high-pollution environments, monitoring air-quality information where available, and seeking timely healthcare for persistent respiratory symptoms. In Nigeria, limited awareness, dependence on polluting household energy sources, traffic-related exposure, occupational exposure, open waste burning, and limited access to air-quality information may affect residents' ability to adopt appropriate preventive practices. Effective air pollution awareness may improve knowledge and encourage the adoption of behaviours that reduce respiratory health risks. Against this background, this study investigates the effect of air pollution awareness on respiratory disease prevention practices among urban residents in Nigeria. The study is anchored on the Health Belief Model, Social Cognitive Theory, and Knowledge-Attitude-Practice framework. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action, providing a basis for understanding residents' decisions to adopt respiratory disease prevention practices. Social Cognitive Theory emphasizes the interaction between knowledge, self-efficacy, social influences, behavioural capabilities, and environmental conditions in shaping health behaviour. The Knowledge-Attitude-Practice framework provides a basis for examining how improved knowledge and attitudes resulting from air pollution awareness may influence preventive practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how air pollution awareness may influence respiratory disease prevention practices among urban residents in Nigeria. The study will adopt a quantitative quasi-experimental research design. Primary data will be collected from adult residents of selected urban communities in Nigeria using a structured interviewer-administered questionnaire and, where feasible, an environmental observation checklist. A multistage sampling technique will be employed to select states, local government areas, urban communities, households, and eligible residents. Where feasible, selected communities or participant groups will be assigned to intervention and comparison groups. Residents in the intervention group will receive structured air pollution awareness education covering major sources of indoor and outdoor air pollution, common pollutants, respiratory and other health effects, vulnerable populations, exposure-reduction strategies, household ventilation, smoke avoidance, safer household energy practices, open waste burning, occupational exposure, use of appropriate protective measures, and timely healthcare seeking for respiratory symptoms. Respiratory disease prevention practices will be assessed using indicators such as avoidance of smoke and open burning, reduction of exposure to highly polluted environments, household ventilation practices, safer cooking-energy practices where feasible, appropriate use of protective measures, attention to air-quality information, and timely healthcare seeking for persistent respiratory symptoms. Descriptive statistics will be used to summarize residents' sociodemographic characteristics, air pollution awareness, exposure to education, and respiratory disease prevention practices. Inferential statistical techniques, including chi-square tests, paired-sample tests, independent-sample tests, and multiple regression analysis, will be used to determine the effect of air pollution awareness on respiratory disease prevention practices. Appropriate diagnostic tests will also be conducted to assess the reliability of the research instrument, model assumptions, goodness of fit, and robustness of the findings. The study is expected to find that air pollution awareness has a significant positive effect on respiratory disease prevention practices among urban residents in Nigeria. Residents exposed to structured air pollution awareness education are expected to demonstrate improved knowledge of pollution sources, exposure risks, and respiratory health consequences. Education may encourage residents to reduce exposure to smoke and other pollutants, improve household ventilation, avoid open waste burning, adopt safer household energy practices where feasible, use appropriate protective measures in highly polluted environments, and seek timely healthcare for persistent respiratory symptoms. However, awareness alone may not guarantee sustained preventive practices because residents may continue to face structural barriers such as limited access to clean household energy, high costs of alternative energy sources, traffic-related pollution, industrial emissions, inadequate waste-management systems, and limited availability of reliable air-quality information. Consequently, air pollution awareness is expected to be most effective when supported by environmental regulations, improved waste management, cleaner household energy options, effective urban planning, air-quality monitoring, and accessible healthcare services. The study is expected to contribute to the literature on environmental health, air pollution, respiratory disease prevention, health education, urban health, and public health in Nigeria by providing empirical evidence on the effect of air pollution awareness on respiratory disease prevention practices among urban residents. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Environment, state environmental protection agencies, local government authorities, urban health programmes, healthcare providers, environmental health professionals, development partners, and policymakers regarding strategies for reducing pollution-related respiratory health risks. The study will also provide evidence-based recommendations for strengthening public air pollution education, improving access to air-quality information, promoting cleaner household energy use, strengthening environmental regulation and waste management, improving urban air-quality monitoring, and encouraging respiratory disease prevention practices among urban residents across Nigeria.
