Effect of Adolescent-Friendly Health Services on Reproductive Health Service Utilization among Adolescents in Nigeria

 

Abstract

Adolescence is a critical stage of physical, emotional, and social development during which individuals require accessible, acceptable, confidential, and appropriate reproductive health information and services. However, adolescents in Nigeria may experience barriers to reproductive health service utilization, including concerns about privacy and confidentiality, stigma, judgmental attitudes from healthcare providers, inconvenient service hours, inadequate information, financial constraints, and limited availability of services designed specifically for their needs. Adolescent-friendly health services are health services designed and delivered in ways that are accessible, equitable, acceptable, appropriate, effective, and responsive to the specific needs of adolescents. Such services may include reproductive health counselling, family planning, sexually transmitted infection prevention and management, HIV testing and counselling, menstrual health support, pregnancy-related services, and sexual and reproductive health education. Reproductive health service utilization refers to adolescents' actual use of available reproductive health services, including seeking counselling, contraception, sexually transmitted infection services, HIV testing, and other appropriate reproductive healthcare. Strengthening adolescent-friendly health services may reduce barriers to service access and encourage adolescents to seek and utilize reproductive health services when needed. Against this background, this study investigates the effect of adolescent-friendly health services on reproductive health service utilization among adolescents in Nigeria. The study will be anchored on Andersen's Behavioral Model of Health Services Use, the Health Belief Model, and the Theory of Planned Behavior. Andersen's Behavioral Model explains healthcare utilization through predisposing, enabling, and need-related factors and provides a framework for understanding how service characteristics may influence adolescents' use of healthcare services. The Health Belief Model explains health-seeking behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action. The Theory of Planned Behavior explains behavioural intention and action through attitudes, subjective norms, and perceived behavioural control. Collectively, these theoretical perspectives provide a suitable framework for explaining how adolescent-friendly health services may influence reproductive health service utilization among adolescents in Nigeria. The study will adopt a quantitative cross-sectional research design. Data will be collected from adolescents aged approximately 10–19 years attending selected healthcare facilities and residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, communities, and eligible adolescents. Adolescent-friendly health services will be assessed using indicators such as privacy and confidentiality, respectful treatment by healthcare providers, convenient operating hours, affordability, accessibility, availability of adolescent-specific information, availability of reproductive health services, trained healthcare personnel, appropriate service environment, non-discriminatory care, and availability of counselling services. Reproductive health service utilization will be assessed using indicators such as use of reproductive health counselling, contraceptive services, sexually transmitted infection services, HIV testing and counselling, menstrual health services, pregnancy-related services, and reproductive health information. Structured questionnaires will be used for data collection, while facility assessments may be conducted to determine the availability and characteristics of adolescent-friendly services. Descriptive statistics will be used to summarize participants' characteristics, availability and accessibility of adolescent-friendly services, and reproductive health service utilization patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and multiple regression analysis, will be used to determine the effect of adolescent-friendly health services on reproductive health service utilization. Diagnostic tests will also be conducted to assess the reliability of the research instrument and the robustness of the findings. The study is expected to find that adolescent-friendly health services have a significant positive effect on reproductive health service utilization among adolescents in Nigeria. Adolescents who have access to confidential, respectful, affordable, accessible, and appropriately designed health services are expected to demonstrate greater utilization of reproductive health services than adolescents with limited access to such services. Privacy and confidentiality may increase adolescents' willingness to discuss sensitive reproductive health concerns with healthcare providers. Convenient service hours and accessible locations may reduce practical barriers to healthcare utilization, while respectful and non-judgmental treatment may increase adolescents' trust in healthcare providers. Availability of appropriate reproductive health information and counselling may also encourage adolescents to seek professional services and make informed health decisions. Conversely, services that are difficult to access, costly, inconvenient, judgmental, or lacking in privacy may discourage adolescents from seeking reproductive healthcare. However, adolescent-friendly health services alone may not fully determine utilization because parental influence, peer norms, cultural beliefs, socioeconomic circumstances, health knowledge, perceived need, and availability of services may also affect adolescents' healthcare-seeking behaviour. The study is expected to contribute to the literature on adolescent-friendly health services, reproductive health service utilization, adolescent health, sexual and reproductive health, health-seeking behaviour, community health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, primary healthcare authorities, healthcare facilities, adolescent health programme managers, healthcare workers, schools, youth organizations, development partners, and policymakers regarding strategies for improving adolescent access to reproductive healthcare. The study will also provide evidence-based recommendations for expanding adolescent-friendly health services, strengthening healthcare-worker training, improving privacy and confidentiality, extending convenient service hours, reducing financial barriers, increasing adolescent-focused health information, improving service accessibility, and integrating comprehensive adolescent reproductive health services into primary healthcare and community health programmes across Nigeria.
