Influence of Workplace Mental Health Screening on Depression Detection among Employees in Nigeria

 

Abstract

Depression remains an important public health concern among employees in Nigeria and may negatively affect psychological well-being, productivity, concentration, work performance, absenteeism, interpersonal relationships, and overall quality of life. Employees may experience depression in association with occupational stress, excessive workloads, job insecurity, workplace conflict, financial pressures, poor work-life balance, and other personal or occupational challenges. Depression may remain undetected because symptoms can be overlooked, normalized, or attributed to work-related fatigue, while stigma, fear of discrimination, concerns about confidentiality, limited awareness, and inadequate access to mental health services may discourage employees from seeking professional help. Early detection of depression is essential for timely psychological assessment, counselling, appropriate treatment, workplace support, and referral to mental health professionals where necessary. Workplace mental health screening provides an opportunity to identify employees experiencing depressive symptoms through organized employee wellness programmes, occupational health services, periodic health assessments, and workplace mental health initiatives. However, limited availability of trained mental health professionals, inadequate workplace mental health programmes, concerns about confidentiality, stigma, low employee participation, and weak referral and follow-up systems may limit the effectiveness of workplace mental health screening in Nigeria. Against this background, this study investigates the influence of workplace mental health screening on depression detection among employees in Nigeria. The study will be anchored on the Health Belief Model, Job Demands-Resources Model, and Occupational Health Theory. The Health Belief Model explains how employees' perceptions of susceptibility to depression, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in workplace mental health screening. The Job Demands-Resources Model explains how excessive job demands and inadequate workplace resources may influence employees' psychological well-being and contribute to work-related stress and depressive symptoms. Occupational Health Theory emphasizes the protection and promotion of workers' physical, psychological, and social well-being through preventive health measures and healthy working environments. Collectively, these theoretical perspectives provide a suitable framework for explaining how workplace mental health screening may influence depression detection among employees in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise employees working in selected public and private organizations across Nigeria. A multistage sampling technique will be used to select states, organizations, institutions, departments, and eligible employees. Workplace mental health screening will be measured using indicators such as availability of workplace mental health screening programmes, screening coverage, frequency of screening activities, use of validated depression screening instruments, availability of trained mental health personnel, confidentiality procedures, employee wellness services, mental health education, counselling services, and referral arrangements. Depression detection will be assessed using indicators such as positive depression screening results, severity of depressive symptoms, newly identified suspected cases, previous undiagnosed depression, referral for professional mental health assessment, confirmed diagnoses where appropriate, and linkage to counselling, treatment, or other mental health support services. Data will be collected using structured questionnaires, validated depression screening instruments, workplace screening registers, occupational health records, counselling records, referral registers, and relevant organizational documents. Descriptive statistics will be used to summarize employees' characteristics, screening coverage, depressive symptoms, and patterns of depression detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of workplace mental health screening on depression detection. Where appropriate, depression detection rates before and after implementation of workplace mental health screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that workplace mental health screening has a significant positive influence on depression detection among employees in Nigeria. Employees who participate in workplace mental health screening are expected to have a higher likelihood of having previously undetected depressive symptoms identified than employees without access to such services. Systematic screening may facilitate early identification of depressive symptoms and provide opportunities for counselling, psychological support, professional assessment, treatment, workplace adjustments, and continued monitoring. Providing mental health screening within the workplace may reduce barriers associated with seeking mental health services outside working hours and may increase access among employees who may otherwise not seek help. Confidential screening may also encourage employees to disclose psychological difficulties that they might otherwise conceal because of stigma or fear of workplace discrimination. Conversely, concerns about confidentiality, stigma, inadequate screening coverage, shortage of trained mental health personnel, low employee participation, and weak referral and follow-up systems may reduce the effectiveness of workplace mental health screening. The study therefore expects accessible, confidential, and well-organized workplace mental health screening to contribute significantly to improved detection and early management of depression among employees in Nigeria. The study is expected to contribute to the literature on workplace mental health screening, depression detection, employee mental health, occupational health, workplace wellness, psychological well-being, preventive healthcare, mental health promotion, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, employers, human resource managers, occupational health practitioners, mental health professionals, workplace counsellors, employee assistance programmes, trade unions, development partners, and policymakers regarding strategies for improving mental health among employees. The study will also provide evidence-based recommendations for expanding workplace mental health screening programmes, strengthening confidential screening and counselling services, increasing access to trained mental health professionals, reducing stigma and discrimination, improving employee mental health education, strengthening referral and follow-up systems, promoting supportive workplace environments, and integrating depression screening into comprehensive occupational health and employee wellness programmes across Nigeria.
