Influence of Trachoma Treatment Programmes on Trachoma Prevalence among Residents of Endemic Communities in Nigeria

Abstract

Trachoma is a neglected tropical eye disease caused by repeated infection with Chlamydia trachomatis and remains an important public health concern in some endemic communities. Repeated infection can result in chronic inflammation and progressive damage to the eyelid and cornea, potentially leading to visual impairment and blindness if left untreated. Trachoma transmission is associated with inadequate access to clean water, poor sanitation, limited facial cleanliness, overcrowding, and insufficient access to appropriate healthcare services. Trachoma treatment programmes form an important component of disease-control strategies and may include community-based antibiotic distribution, identification and treatment of affected individuals, health education, facial cleanliness promotion, environmental sanitation activities, and referral for surgical management of advanced disease. Trachoma prevalence refers to the proportion of individuals within a defined population who have the disease or specified clinical signs of trachoma during a particular period. In Nigeria, sustained treatment and control programmes are important for reducing transmission and preventing progression to severe disease in endemic communities. Against this background, this study investigates the influence of trachoma treatment programmes on trachoma prevalence among residents of endemic communities in Nigeria. The study will be anchored on the Health Belief Model, the Health Systems Framework, and the Social Ecological Model. The Health Belief Model explains preventive health behaviour through perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and cues to action and provides a framework for understanding community participation in trachoma treatment and prevention activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, essential medicines, financing, and accessibility as important components of effective disease-control programmes. The Social Ecological Model recognizes that disease transmission and health behaviour are influenced by interacting individual, household, community, environmental, and health-system factors. Collectively, these theoretical perspectives provide a suitable framework for explaining how trachoma treatment programmes may influence trachoma prevalence in endemic Nigerian communities. The study will adopt a quantitative cross-sectional or repeated cross-sectional research design, depending on the availability of programme and surveillance data. Data will be collected from residents of selected communities identified as trachoma-endemic in Nigeria. A multistage sampling technique will be used to select states, local government areas, endemic communities, households, and eligible participants. Trachoma treatment programmes will be assessed using indicators such as availability and coverage of antibiotic treatment, frequency of treatment rounds, proportion of eligible residents reached, treatment adherence, availability of trained healthcare workers, community mobilization, health education, facial cleanliness promotion, environmental sanitation activities, referral services, and programme continuity. Trachoma prevalence will be assessed using standardized clinical examination and appropriate programme surveillance methods to determine the proportion of residents presenting with relevant clinical signs of trachoma. Data will be obtained through structured questionnaires, clinical examination, treatment registers, community health-worker records, programme reports, and relevant surveillance records where available. Descriptive statistics will be used to summarize participants' characteristics, programme coverage, and trachoma prevalence patterns. Inferential statistical techniques, including chi-square tests, correlation analysis, and regression analysis, will be used to determine the influence of trachoma treatment programmes on trachoma prevalence. Where longitudinal programme data are available, prevalence before and after treatment interventions will be compared to assess changes over time. Diagnostic tests will also be conducted to assess the reliability and robustness of the findings. The study is expected to find that effective and adequately implemented trachoma treatment programmes have a significant negative influence on trachoma prevalence among residents of endemic communities in Nigeria. Communities with higher treatment coverage and consistent implementation of control interventions are expected to have lower trachoma prevalence than communities with limited programme coverage. Community-based antibiotic treatment may reduce the reservoir of infection and interrupt transmission, while health education and facial cleanliness promotion may encourage behaviours that reduce exposure and reinfection. Environmental sanitation and improvements in access to water may further support sustainable reductions in transmission. Conversely, inadequate treatment coverage, poor adherence, medicine shortages, limited community participation, weak surveillance, poor sanitation, and inadequate access to clean water may reduce programme effectiveness and allow transmission to persist. However, trachoma treatment programmes alone may not fully determine disease prevalence because environmental conditions, household crowding, water availability, sanitation practices, hygiene behaviour, population movement, and underlying transmission patterns may also influence trachoma prevalence. The study is expected to contribute to the literature on trachoma treatment programmes, trachoma prevalence, neglected tropical diseases, community health, infectious disease control, eye health, epidemiology, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, neglected tropical disease programme managers, local government health authorities, eye-care professionals, community health workers, development partners, and policymakers regarding strategies for strengthening trachoma elimination efforts. The study will also provide evidence-based recommendations for improving treatment coverage, strengthening community mobilization, ensuring adequate availability of medicines, improving treatment monitoring, expanding health education, promoting facial cleanliness, strengthening water and sanitation interventions, improving surveillance, and integrating trachoma control activities into broader neglected tropical disease and primary healthcare programmes across endemic communities in Nigeria.
