Influence of Community-Based Reproductive Health Assessment on Reproductive Health Problems Detection among Women of Reproductive Age in Nigeria

Abstract

Reproductive health is an important component of women's overall health and well-being, yet many reproductive health problems among women of reproductive age in Nigeria may remain undetected because of limited access to preventive healthcare, inadequate awareness, financial constraints, geographical barriers, stigma, and insufficient availability of reproductive health services. Reproductive health problems may include menstrual abnormalities, reproductive tract infections, sexually transmitted infections, infertility-related concerns, cervical abnormalities, contraceptive-related problems, and other conditions affecting reproductive health and functioning. Delayed detection may result in complications, reduced quality of life, impaired fertility, adverse pregnancy outcomes, and increased healthcare burdens. Community-based reproductive health assessment provides an opportunity to identify reproductive health concerns through community outreach programmes, primary healthcare centres, mobile health services, women's health initiatives, and other accessible community platforms. Early identification may facilitate appropriate counselling, laboratory or clinical assessment, treatment, referral, and follow-up. However, limited service availability, shortage of trained healthcare personnel, inadequate privacy, cultural barriers, low participation, and weak referral systems may reduce the effectiveness of community-based reproductive health assessment in Nigeria. Against this background, this study investigates the influence of community-based reproductive health assessment on reproductive health problem detection among women of reproductive age in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and the Health Systems Framework. The Health Belief Model explains how women's perceptions of susceptibility to reproductive health problems, perceived severity, perceived benefits of assessment, perceived barriers, and cues to action may influence participation in community-based reproductive health assessment. The Social Ecological Model emphasizes the influence of individual, interpersonal, household, community, socioeconomic, cultural, and environmental factors on women's reproductive health behaviours, healthcare-seeking practices, and access to preventive services. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, privacy, and accessibility as essential components of effective community-based reproductive health assessment and referral programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based reproductive health assessment may influence reproductive health problem detection among women of reproductive age in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise women aged 15 to 49 years residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible women. Community-based reproductive health assessment will be measured using indicators such as availability of assessment services, assessment coverage, frequency of community assessment activities, menstrual health assessment, reproductive tract infection assessment, sexually transmitted infection risk assessment, contraceptive and family planning assessment, fertility-related assessment, cervical health assessment where appropriate, availability of trained healthcare personnel, privacy and confidentiality arrangements, reproductive health education, and referral services. Reproductive health problem detection will be assessed using indicators such as identified menstrual abnormalities, reproductive tract infections, sexually transmitted infections, cervical abnormalities, contraceptive-related concerns, fertility-related problems, reproductive symptoms requiring further evaluation, newly detected reproductive health problems, referrals for clinical or laboratory assessment, confirmed conditions where appropriate, and linkage to appropriate treatment and reproductive healthcare services. Data will be collected using structured questionnaires, community reproductive health assessment forms, screening registers, laboratory records, healthcare facility records, referral registers, and relevant community health programme documents. Descriptive statistics will be used to summarize participants' characteristics, reproductive health factors, assessment coverage, clinical findings, and patterns of reproductive health problem detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the influence of community-based reproductive health assessment on reproductive health problem detection. Where appropriate, detection rates before and after implementation of community-based reproductive health assessment activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based reproductive health assessment has a significant positive influence on reproductive health problem detection among women of reproductive age in Nigeria. Women who participate in community-based reproductive health assessment are expected to have a higher likelihood of having previously unrecognized reproductive health problems identified than women without access to such services. Community-based assessment may facilitate early recognition of menstrual abnormalities, reproductive tract infections, sexually transmitted infections, cervical health concerns, fertility-related problems, and other reproductive health conditions and provide opportunities for appropriate clinical evaluation and management. Bringing reproductive health services closer to women may reduce geographical, financial, and time-related barriers and encourage women who may not routinely attend healthcare facilities to participate in preventive assessments. Community-based reproductive health education may also improve women's knowledge of reproductive health, warning signs, infection prevention, family planning, cervical health, and appropriate healthcare-seeking behaviour. Early detection may support timely treatment, referral, follow-up, and prevention of avoidable complications. Conversely, inadequate assessment coverage, limited privacy, shortage of trained healthcare workers, stigma, cultural barriers, low participation, financial constraints, and weak referral and follow-up systems may reduce the effectiveness of community-based reproductive health assessment. The study therefore expects accessible, confidential, and well-organized community-based reproductive health assessment to contribute significantly to improved detection and early management of reproductive health problems among women of reproductive age in Nigeria. The study is expected to contribute to the literature on community-based reproductive health assessment, reproductive health problem detection, women's health, reproductive health services, maternal health, sexual and reproductive healthcare, preventive healthcare, community health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, reproductive health practitioners, nurses, midwives, community health workers, women's health programmes, development partners, and policymakers regarding strategies for strengthening women's reproductive health services. The study will also provide evidence-based recommendations for expanding community-based reproductive health assessment programmes, improving access to reproductive health services, strengthening reproductive health education, ensuring privacy and confidentiality, increasing availability of trained healthcare personnel, improving access to appropriate diagnostic services, strengthening referral and follow-up systems, reducing financial and geographical barriers, and integrating comprehensive reproductive health assessment into appropriate community-based women's health programmes across Nigeria.
