Impact of Workplace Liver Function Screening on Liver Disease Detection among Industrial Workers in Nigeria
Abstract
Liver diseases remain an important occupational and public health concern among industrial workers in Nigeria because workers may be exposed to substances and workplace conditions that can adversely affect liver health. Industrial workers may encounter chemicals, solvents, heavy metals, fumes, dust, pesticides, petroleum products, and other potentially hepatotoxic substances during their occupational activities. In addition, alcohol consumption, viral hepatitis infection, obesity, diabetes, medication use, and other non-occupational factors may contribute to liver disease among workers. Some liver diseases may develop gradually without noticeable symptoms, resulting in delayed detection and continued exposure to factors that may worsen liver damage. Undetected liver disease can progress to chronic liver injury, fibrosis, cirrhosis, liver failure, and other serious complications, potentially affecting workers' health, productivity, safety, and quality of life. Workplace liver function screening provides an opportunity to identify abnormal liver function among workers through periodic occupational health assessments, laboratory testing, employee wellness programmes, and workplace medical examinations. Early identification may facilitate further clinical evaluation, reduction of occupational exposure, lifestyle modification, appropriate treatment, and referral to specialist services where necessary. However, inadequate occupational health programmes, limited laboratory facilities, shortage of trained healthcare personnel, irregular screening schedules, high screening costs, low worker participation, and weak referral and follow-up systems may limit the effectiveness of workplace liver function screening in Nigeria. Against this background, this study investigates the impact of workplace liver function screening on liver disease detection among industrial workers in Nigeria. The study will be anchored on Occupational Health Theory, the Health Belief Model, and the Health Systems Framework. Occupational Health Theory emphasizes the prevention, identification, monitoring, and management of workplace-related health risks and the promotion of workers' health and safety. The Health Belief Model explains how industrial workers' perceptions of susceptibility to liver disease, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in workplace liver function screening. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective occupational health screening and disease detection programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how workplace liver function screening may influence liver disease detection among industrial workers in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise industrial workers employed in selected manufacturing, chemical, petroleum, agricultural processing, mining, pharmaceutical, and other industrial facilities across Nigeria. A multistage sampling technique will be used to select states, industrial zones, organizations, factories, departments, and eligible workers. Workplace liver function screening will be measured using indicators such as availability of workplace liver health screening programmes, screening coverage, frequency of screening activities, liver function testing, assessment of occupational exposure to potentially hepatotoxic substances, hepatitis screening where appropriate, availability of laboratory facilities, trained healthcare personnel, health education, occupational exposure monitoring, and referral arrangements. Liver disease detection will be assessed using indicators such as abnormal liver enzyme results, abnormal liver function findings, positive hepatitis screening results where applicable, suspected liver disease, newly identified liver abnormalities, confirmed liver disease following appropriate clinical evaluation, referrals for further assessment, and linkage to treatment and follow-up care. Data will be collected using structured questionnaires, workplace screening registers, laboratory reports, occupational health records, liver function assessment forms, referral registers, and relevant organizational documents. Descriptive statistics will be used to summarize workers' characteristics, occupational exposures, screening coverage, liver function findings, and patterns of liver disease detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of workplace liver function screening on liver disease detection. Where appropriate, liver disease detection rates before and after implementation of workplace screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that workplace liver function screening has a significant positive impact on liver disease detection among industrial workers in Nigeria. Industrial workers who participate in workplace liver function screening are expected to have a higher likelihood of having previously undetected liver abnormalities identified than workers without access to routine workplace screening. Periodic assessment of liver function may facilitate early identification of abnormal liver enzymes and other findings requiring further clinical investigation. Early detection may provide opportunities for confirmatory testing, medical evaluation, treatment, lifestyle modification, monitoring, and reduction or control of occupational exposure to potentially hepatotoxic substances. Workplace screening may also increase workers' awareness of liver disease risk factors and encourage healthier behaviours and timely healthcare utilization. Conversely, inadequate screening coverage, limited laboratory capacity, shortage of trained healthcare personnel, high screening costs, low worker participation, and weak referral and follow-up systems may reduce the effectiveness of workplace liver function screening. The study therefore expects regular and comprehensive workplace liver function screening to contribute significantly to improved detection and early management of liver disease among industrial workers in Nigeria. The study is expected to contribute to the literature on workplace liver function screening, liver disease detection, industrial worker health, occupational liver health, occupational exposure, workplace health promotion, preventive healthcare, occupational medicine, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, employers, industrial organizations, occupational health practitioners, medical laboratories, healthcare providers, hepatologists, workplace safety officers, development partners, and policymakers regarding strategies for improving liver health among industrial workers. The study will also provide evidence-based recommendations for expanding workplace liver function screening programmes, improving access to laboratory testing, strengthening occupational exposure assessment and monitoring, increasing worker awareness of liver disease prevention, improving availability of trained healthcare and laboratory personnel, strengthening referral and follow-up systems, promoting appropriate workplace safety measures, and integrating liver health assessment into comprehensive occupational health programmes across Nigeria.
