Abstract

Hypertension remains a major public health concern among adults in Nigeria and is an important risk factor for cardiovascular disease, stroke, heart failure, kidney disease, and premature mortality. Factory workers may be exposed to several factors that can increase the risk of hypertension, including physical inactivity, unhealthy dietary practices, occupational stress, prolonged working hours, shift work, exposure to workplace hazards, tobacco use, alcohol consumption, overweight, and obesity. Hypertension often develops without noticeable symptoms, resulting in delayed diagnosis and treatment among individuals who may consider themselves healthy. Undetected and poorly controlled hypertension can progressively damage vital organs and increase the risk of severe cardiovascular and renal complications. Early detection is therefore essential for initiating appropriate treatment, lifestyle modification, monitoring, and preventive interventions. Workplace hypertension screening provides an opportunity to bring blood pressure assessment and related health services directly to factory workers through occupational health programmes, periodic medical examinations, workplace health campaigns, and employee wellness initiatives. However, inadequate screening coverage, limited occupational health facilities, shortages of trained healthcare personnel, irregular screening schedules, low worker participation, concerns about confidentiality, and weak referral systems may limit the effectiveness of workplace hypertension screening programmes in Nigeria. Against this background, this study investigates the impact of workplace hypertension screening on hypertension detection among factory workers in Nigeria. The study will be anchored on the Health Belief Model, Occupational Health Theory, and the Health Systems Framework. The Health Belief Model explains how factory workers' perceptions of susceptibility to hypertension, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in workplace hypertension screening. Occupational Health Theory emphasizes the prevention, identification, monitoring, and management of health risks associated with the workplace and the promotion of workers' health and safety. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective occupational healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how workplace hypertension screening may influence hypertension detection among factory workers in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise factory workers employed in selected manufacturing and industrial facilities across Nigeria. A multistage sampling technique will be used to select states, industrial areas, factories, departments, and eligible workers. Workplace hypertension screening will be measured using indicators such as availability of workplace screening programmes, screening coverage, frequency of blood pressure assessment, availability of trained healthcare personnel, availability of blood pressure monitoring equipment, health education, cardiovascular risk assessment, employee participation, and referral arrangements. Hypertension detection will be assessed using indicators such as elevated blood pressure readings, newly identified suspected hypertension cases, confirmed hypertension following appropriate repeated clinical assessment, blood pressure classification, identification of workers with previously undiagnosed hypertension, referral for further medical evaluation, and initiation of appropriate management or follow-up. Data will be collected using structured questionnaires, workplace screening registers, occupational health records, blood pressure assessment records, medical examination records, referral registers, and relevant organizational documents. Descriptive statistics will be used to summarize workers' characteristics, screening coverage, blood pressure findings, and patterns of hypertension detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of workplace hypertension screening on hypertension detection. Where appropriate, hypertension detection rates before and after implementation of workplace screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that workplace hypertension screening has a significant positive impact on hypertension detection among factory workers in Nigeria. Factory workers who participate in workplace hypertension screening are expected to have a higher likelihood of having previously undiagnosed hypertension identified than workers without access to regular screening. Routine blood pressure assessment may facilitate early identification of elevated blood pressure and provide opportunities for confirmatory evaluation, lifestyle modification, nutritional counselling, medical treatment, and continuous monitoring. Conducting screening within factories may reduce time and accessibility barriers associated with seeking preventive healthcare outside working hours and may encourage workers who rarely visit healthcare facilities to undergo routine health assessments. Workplace hypertension screening may also increase workers' awareness of hypertension risk factors and encourage healthier behaviours. Conversely, inadequate screening coverage, limited occupational health resources, shortage of trained personnel, low worker participation, concerns about confidentiality, and weak referral and follow-up systems may reduce the effectiveness of workplace hypertension screening. The study therefore expects accessible, regular, and well-organized workplace hypertension screening to contribute significantly to improved detection and early management of hypertension among factory workers in Nigeria. The study is expected to contribute to the literature on workplace hypertension screening, hypertension detection, factory worker health, occupational health, cardiovascular disease prevention, employee wellness, workplace health promotion, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Labour and Employment, employers, manufacturing organizations, occupational health practitioners, healthcare providers, workplace health and safety officers, trade unions, development partners, and policymakers regarding strategies for improving cardiovascular health among factory workers. The study will also provide evidence-based recommendations for expanding workplace hypertension screening programmes, improving access to regular blood pressure assessment, strengthening occupational health services, increasing worker awareness of hypertension and its risk factors, ensuring availability of trained healthcare personnel and appropriate screening equipment, protecting the confidentiality of screening results, strengthening referral and follow-up systems, promoting healthy workplace environments, and integrating hypertension screening into comprehensive occupational health and workplace wellness programmes across Nigeria.
