Impact of University Counselling Services on Mental Health Service Utilization among Undergraduates in Nigeria

Abstract

Mental health problems among university students represent an important public health and educational concern because psychological difficulties may affect students' academic performance, social relationships, emotional wellbeing, and overall university experience. Undergraduates in Nigeria may experience academic pressure, financial difficulties, relationship problems, family challenges, social adjustment difficulties, and uncertainty about future employment, which may increase their need for mental health support. University counselling services can provide accessible opportunities for psychological assessment, counselling, emotional support, stress management, crisis intervention, referral, and other mental health interventions. However, stigma, limited awareness of available services, concerns about confidentiality, financial constraints, inadequate counselling personnel, inconvenient service hours, and negative perceptions of counselling may discourage students from utilizing available mental health services. Against this background, this study investigates the impact of university counselling services on mental health service utilization among undergraduates in Nigeria. The study will be anchored on the Health Belief Model, Andersen's Behavioral Model of Health Services Use, and Social Cognitive Theory. The Health Belief Model explains how students' perceptions of mental health needs, perceived benefits of counselling, perceived barriers, and cues to action may influence their utilization of mental health services. Andersen's Behavioral Model explains healthcare utilization in terms of predisposing characteristics, enabling resources, and perceived or actual need for services, providing a suitable framework for examining factors associated with students' use of university counselling services. Social Cognitive Theory emphasizes the roles of knowledge, self-efficacy, social interaction, observational learning, and environmental influences in shaping health-seeking behaviours. Collectively, these theoretical perspectives provide a suitable framework for explaining how university counselling services may influence mental health service utilization among undergraduates in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise undergraduate students enrolled in selected public and private universities across Nigeria. A multistage sampling technique will be used to select states, universities, faculties, departments, and eligible undergraduates. University counselling services will be measured using indicators such as availability of counselling centres, accessibility of counselling services, frequency of service availability, range of services provided, availability of trained counsellors, counselling appointment procedures, confidentiality arrangements, individual counselling, group counselling, psychological assessment, crisis support, stress management services, referral services, follow-up arrangements, online counselling options, service hours, and awareness of available counselling services. Mental health service utilization will be assessed using indicators such as attendance at university counselling sessions, frequency of counselling visits, utilization of psychological assessment services, participation in group counselling, use of crisis-support services, utilization of referral services, participation in mental health education programmes, use of online counselling where available, follow-up attendance, and continued engagement with appropriate mental health services. Data will be collected using structured questionnaires, counselling service utilization records where ethically and practically accessible, and appropriately validated mental health service utilization instruments. Descriptive statistics will be used to summarize students' demographic and academic characteristics, awareness and accessibility of counselling services, and patterns of mental health service utilization. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the impact of university counselling services on mental health service utilization. Where appropriate, comparisons between students with access to functional counselling services and those with limited access may also be conducted. Diagnostic tests will be performed to assess the reliability, validity, and robustness of the findings. The study is expected to find that accessible and functional university counselling services have a significant positive impact on mental health service utilization among undergraduates in Nigeria. Students with greater access to university counselling services are expected to demonstrate higher utilization of mental health services than students with limited access to such services. Accessible counselling centres, qualified counsellors, confidential service delivery, flexible appointment arrangements, individual and group counselling, crisis support, referral systems, and follow-up services may encourage students to seek and continue using appropriate mental health support. Increased awareness of counselling services may also reduce uncertainty and improve students' willingness to seek assistance when experiencing psychological difficulties. However, stigma, fear of being judged, confidentiality concerns, limited counselling personnel, inadequate funding, inconvenient service schedules, long waiting periods, weak referral systems, and negative perceptions of mental health services may reduce utilization. The study therefore expects accessible, confidential, student-friendly, adequately staffed, and well-integrated university counselling services to contribute significantly to improved mental health service utilization among undergraduates in Nigeria. The study is expected to contribute to the literature on university counselling services, mental health service utilization, undergraduate mental health, student counselling, mental health promotion, help-seeking behaviour, university health services, and public health in Nigeria. The findings will provide useful information to the National Universities Commission, Federal Ministry of Education, Federal Ministry of Health and Social Welfare, university administrators, student affairs departments, counselling centres, university health services, psychologists, counsellors, psychiatrists, social workers, public health practitioners, student organizations, development partners, and policymakers regarding strategies for improving students' access to mental health support. The study will also provide evidence-based recommendations for strengthening university counselling centres, increasing the availability of qualified counsellors, improving confidentiality and referral systems, expanding flexible and online counselling options, integrating mental health services into university health systems, increasing student awareness, reducing stigma, and establishing sustainable mental health support programmes across universities in Nigeria.
