Impact of School-Based First-Aid Services on First-Aid Response among Secondary School Students in Nigeria

Abstract

First-aid response is an important component of school health and emergency preparedness because students may experience injuries, sudden illnesses, sports-related accidents, burns, falls, bleeding, and other health emergencies during school activities. Prompt and appropriate first-aid response can help reduce the severity of injuries, prevent complications, and provide temporary care while awaiting professional medical attention. However, many secondary schools in Nigeria may face challenges such as limited availability of first-aid supplies, inadequate first-aid training, absence of designated first-aid personnel, poor emergency preparedness, limited knowledge of first-aid procedures, and weak referral arrangements. School-based first-aid services provide organized mechanisms for preventing and responding to health emergencies within the school environment through first-aid kits, trained staff, first-aid rooms, emergency protocols, student health education, and referral systems. Effective school-based first-aid services may therefore improve students' ability to recognize emergencies and provide appropriate immediate assistance. Against this background, this study investigates the impact of school-based first-aid services on first-aid response among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and the Social Ecological Model. The Health Belief Model explains how students' perceptions of the seriousness of injuries, perceived benefits of first-aid actions, perceived barriers, and cues to action may influence their willingness to respond during emergencies. Social Cognitive Theory emphasizes the importance of knowledge, observational learning, self-efficacy, and environmental factors in developing and applying first-aid behaviours. The Social Ecological Model recognizes that first-aid response among students is influenced by individual knowledge and skills, peer and teacher support, school policies, availability of first-aid resources, and broader community emergency-care systems. Collectively, these theoretical perspectives provide a suitable framework for explaining how school-based first-aid services may influence first-aid response among secondary school students in Nigeria. The study will adopt a quantitative quasi-experimental research design. The study population will comprise students enrolled in selected public and private secondary schools in Nigeria, as well as teachers, school health personnel, and designated first-aid providers involved in school-based emergency care. A multistage sampling technique will be used to select states, local government areas, communities, schools, classes, and eligible students. School-based first-aid services will be assessed using indicators such as availability of functional first-aid kits, accessibility of first-aid supplies, availability of designated first-aid rooms or treatment areas, presence of trained first-aid personnel, first-aid training programmes, emergency response protocols, injury documentation, emergency communication systems, referral arrangements, first-aid drills, regular inspection and replenishment of first-aid supplies, teacher participation, and student health education. First-aid response will be assessed using indicators such as students' knowledge of first-aid procedures, recognition of common emergencies, ability to control bleeding, appropriate response to burns, management of minor injuries, appropriate response to fainting or unconsciousness, safe handling of fractures and suspected spinal injuries, emergency communication, appropriate referral, willingness to provide first aid, and practical first-aid performance. Data will be collected using structured questionnaires, first-aid knowledge tests, practical skills assessment checklists, school health records, injury registers, first-aid supply inventories, emergency response records, and relevant school documents. Descriptive statistics will be used to summarize students' demographic characteristics, exposure to school-based first-aid services, first-aid knowledge, and response patterns. Inferential statistical techniques, including chi-square tests, paired-sample tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of school-based first-aid services on first-aid response. Where a quasi-experimental design is adopted, first-aid knowledge and practical response before and after implementation of an enhanced school-based first-aid programme may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective school-based first-aid services have a significant positive impact on first-aid response among secondary school students in Nigeria. Students attending schools with accessible first-aid supplies, trained first-aid personnel, structured emergency protocols, regular first-aid education, and practical drills are expected to demonstrate better knowledge, confidence, decision-making, and practical first-aid response than students with limited exposure to school-based first-aid services. Regular training and practical demonstrations may improve students' ability to recognize emergencies, provide appropriate immediate care, communicate with emergency services, and seek professional assistance when necessary. The availability of functional first-aid resources may also support prompt management of minor injuries while reducing delays in accessing appropriate care. However, inadequate resources, poor maintenance of first-aid kits, limited training opportunities, lack of trained personnel, fear of causing harm, low student confidence, and weak emergency referral systems may reduce the effectiveness of school-based first-aid services. The study therefore expects accessible, adequately equipped, practical, and regularly maintained school-based first-aid services to contribute significantly to improved first-aid response among secondary school students in Nigeria. The study is expected to contribute to the literature on school-based first aid, first-aid response, student health, emergency preparedness, school health services, injury prevention, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, state ministries of health and education, school management authorities, teachers, school health personnel, emergency-care providers, community health workers, development partners, and policymakers regarding strategies for strengthening emergency preparedness in schools. The study will also provide evidence-based recommendations for establishing functional first-aid systems in secondary schools, ensuring adequate first-aid supplies, training teachers and students in basic first aid, conducting regular practical drills, strengthening emergency communication and referral systems, maintaining first-aid equipment, documenting school injuries, and integrating first-aid education into school health and safety programmes across Nigeria.
