Impact of Personal Protective Equipment Provision on Occupational Safety Compliance among Healthcare Workers in Nigeria

 

Abstract

Occupational safety is an important public health concern in healthcare settings because healthcare workers may be exposed to biological, chemical, physical, and other workplace hazards during the provision of health services. Healthcare workers may face risks of exposure to infectious agents, blood and body fluids, hazardous chemicals, sharps injuries, and other occupational hazards. Personal protective equipment (PPE), including gloves, protective gowns, face masks, respirators, eye protection, and protective footwear, provides an important barrier against workplace exposure when appropriately selected, available, and correctly used. However, inadequate PPE availability, irregular supply, inappropriate equipment, limited training, discomfort, poor fit, insufficient supervision, and inconsistent enforcement of safety procedures may affect healthcare workers' compliance with occupational safety requirements in Nigerian healthcare facilities. Against this background, this study investigates the impact of personal protective equipment provision on occupational safety compliance among healthcare workers in Nigeria. The study will be anchored on the Social Ecological Model, Health Belief Model, and Hierarchy of Controls Framework. The Social Ecological Model emphasizes the interaction between individual, interpersonal, organizational, environmental, and policy factors in shaping occupational safety behaviour. The Health Belief Model explains how healthcare workers' perceptions of occupational risks, perceived severity of workplace hazards, perceived benefits of PPE use, perceived barriers, and cues to action may influence safety compliance. The Hierarchy of Controls Framework emphasizes the importance of controlling occupational hazards, with PPE serving as an important form of protection when hazards cannot be adequately eliminated, substituted, or controlled through engineering and administrative measures. Collectively, these theoretical perspectives provide a suitable framework for explaining how PPE provision may influence occupational safety compliance among healthcare workers in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise healthcare workers, including doctors, nurses, midwives, laboratory personnel, pharmacists, community health workers, and other eligible healthcare personnel working in selected public and private healthcare facilities in Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, departments, and eligible healthcare workers. PPE provision will be assessed using indicators such as availability, adequacy, accessibility, appropriateness, quality, continuity of supply, range of PPE available, availability of appropriate sizes, replacement of damaged or used equipment, storage conditions, and availability at points of care. Occupational safety compliance will be assessed using indicators such as appropriate PPE selection, correct donning and doffing, consistent PPE use during exposure-prone procedures, adherence to standard precautions, hand hygiene, sharps safety, safe handling of blood and body fluids, appropriate disposal of contaminated materials, compliance with infection prevention procedures, and adherence to facility occupational safety policies. Data will be collected using structured questionnaires, PPE availability checklists, direct observation, occupational health records, facility procurement and stock records, infection prevention and control records, and workplace safety inspection reports. Descriptive statistics will be used to summarize healthcare workers' characteristics, PPE availability, occupational exposure risks, and safety compliance levels. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of PPE provision on occupational safety compliance. Where a quasi-experimental design is adopted, safety compliance before and after improved PPE provision may be compared, or healthcare workers in facilities with adequate PPE provision may be compared with those in facilities with inadequate provision. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that adequate provision of personal protective equipment has a significant positive impact on occupational safety compliance among healthcare workers in Nigeria. Healthcare workers who have consistent access to appropriate, adequate, and properly maintained PPE are expected to demonstrate higher levels of compliance with occupational safety procedures than workers experiencing PPE shortages or inconsistent availability. Adequate PPE provision may reduce practical barriers to compliance and encourage healthcare workers to follow standard precautions during patient care. However, PPE availability alone may not guarantee appropriate use, as inadequate training, discomfort, poor fit, supply interruptions, high workload, inadequate supervision, risk perception, and weak enforcement of occupational safety policies may affect compliance. The study therefore expects adequate and continuous PPE provision, combined with appropriate training, supervision, monitoring, and enforcement of workplace safety procedures, to contribute significantly to improved occupational safety compliance among healthcare workers in Nigeria. The study is expected to contribute to the literature on personal protective equipment, occupational safety compliance, healthcare worker safety, workplace hazards, infection prevention and control, occupational health, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, healthcare facility administrators, occupational health departments, infection prevention and control committees, healthcare professional regulatory bodies, healthcare workers, development partners, and policymakers regarding strategies for strengthening occupational safety. The study will also provide evidence-based recommendations for ensuring consistent PPE availability, strengthening PPE procurement and supply systems, providing appropriate PPE sizes and types, improving healthcare worker training, strengthening supervision and safety monitoring, establishing effective PPE stock management systems, enforcing occupational safety policies, and integrating PPE provision into comprehensive occupational health and infection prevention programmes across healthcare facilities in Nigeria.
