Impact of Patient Feedback Systems on Patient Satisfaction in Nigerian Public Hospitals

 

Abstract

Patient satisfaction is an important indicator of healthcare quality and reflects patients' perceptions of the accessibility, responsiveness, communication, timeliness, and overall quality of healthcare services received. In Nigerian public hospitals, patient dissatisfaction may arise from prolonged waiting times, poor communication, inadequate information, unfriendly staff attitudes, limited responsiveness to patient concerns, overcrowding, and perceived weaknesses in service delivery. Patient feedback systems provide structured mechanisms through which patients can report their experiences, express concerns, make suggestions, and evaluate healthcare services. These systems may include patient satisfaction surveys, suggestion boxes, complaint-management platforms, electronic feedback systems, telephone or SMS feedback channels, patient liaison services, and formal complaint-resolution procedures. Effective collection, analysis, and utilization of patient feedback may enable hospitals to identify service deficiencies and implement improvements that enhance patient experience and satisfaction. Against this background, this study investigates the impact of patient feedback systems on patient satisfaction in Nigerian public hospitals. The study will be anchored on the Donabedian Model of Healthcare Quality, the SERVQUAL Model, and Systems Theory. The Donabedian Model explains healthcare quality through structure, process, and outcome, with patient feedback systems representing an important quality-improvement process and patient satisfaction serving as a measurable outcome. The SERVQUAL Model emphasizes patients' perceptions of service quality across dimensions such as reliability, responsiveness, assurance, empathy, and tangibles, all of which may influence satisfaction with healthcare services. Systems Theory views the hospital as an interconnected system in which patients, healthcare workers, management, feedback mechanisms, service processes, and quality-improvement activities interact to influence healthcare outcomes. Collectively, these theoretical perspectives provide a suitable framework for explaining how patient feedback systems may influence patient satisfaction in Nigerian public hospitals. The study will adopt a quantitative quasi-experimental or analytical comparative research design. The study population will comprise patients receiving services in selected Nigerian public hospitals, as well as hospital managers and healthcare workers responsible for collecting, processing, and responding to patient feedback. A multistage sampling technique will be used to select states, local government areas, public hospitals, departments, service units, healthcare workers, and eligible patients. Patient feedback systems will be assessed using indicators such as availability of formal feedback mechanisms, patient satisfaction surveys, suggestion boxes, complaint-management systems, electronic feedback platforms, SMS or telephone feedback channels, patient liaison services, accessibility of feedback mechanisms, frequency of feedback collection, documentation of complaints, response time, feedback analysis, communication of findings, corrective actions, and monitoring of implemented improvements. Patient satisfaction will be assessed using indicators such as satisfaction with healthcare providers, staff communication, waiting time, accessibility of services, cleanliness, privacy, responsiveness to concerns, quality of care, availability of medicines and diagnostic services, complaint resolution, and overall satisfaction with hospital services. Data will be collected using structured questionnaires, standardized patient satisfaction instruments, patient feedback records, complaint registers, hospital quality-improvement reports, electronic feedback databases where available, and relevant hospital documents. Descriptive statistics will be used to summarize patients' demographic characteristics, hospital service experiences, feedback-system utilization, and satisfaction levels. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of patient feedback systems on patient satisfaction. Where a quasi-experimental design is adopted, patient satisfaction levels before and after implementation of a structured feedback system may be compared. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective patient feedback systems have a significant positive impact on patient satisfaction in Nigerian public hospitals. Patients attending hospitals with accessible and responsive feedback systems are expected to report higher satisfaction with healthcare services than patients attending facilities with limited or ineffective feedback mechanisms. Regular collection and analysis of patient feedback may enable hospital management to identify problems relating to waiting time, staff communication, service accessibility, cleanliness, availability of medicines, and other aspects of patient experience. Prompt responses to complaints and implementation of corrective measures may also improve patients' perceptions of hospital responsiveness and accountability. However, limited awareness of feedback mechanisms, fear of negative consequences, inadequate complaint resolution, poor documentation, delayed responses, weak management commitment, and failure to act on feedback may reduce their effectiveness. The study therefore expects accessible, responsive, transparent, and action-oriented patient feedback systems to contribute significantly to improved patient satisfaction in Nigerian public hospitals. The study is expected to contribute to the literature on patient feedback systems, patient satisfaction, healthcare quality, patient experience, complaint management, health service responsiveness, public hospital management, and patient-centred care in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, public hospital administrators, healthcare workers, patient liaison officers, quality-improvement committees, civil society organizations, development partners, and policymakers regarding strategies for strengthening patient-centred healthcare delivery. The study will also provide evidence-based recommendations for expanding accessible patient feedback mechanisms, strengthening complaint-resolution procedures, improving feedback response times, routinely analysing patient experiences, communicating feedback findings to hospital staff and management, implementing corrective actions, and establishing continuous quality-improvement systems based on patients' experiences across Nigerian public hospitals.
