Impact of Hospital Nutrition Services on Nutritional Care among Hospitalized Patients in Nigeria

 

Abstract

Adequate nutritional care is an important component of quality hospital care because hospitalized patients may have increased nutritional requirements, reduced appetite, difficulty eating, disease-related nutritional deficiencies, or medical conditions that affect their ability to consume and utilize nutrients. Poor nutritional status during hospitalization may contribute to delayed recovery, impaired wound healing, increased susceptibility to infections, prolonged hospital stay, higher healthcare costs, and poorer clinical outcomes. Hospital nutrition services provide mechanisms for assessing patients' nutritional needs, developing appropriate meal plans, monitoring dietary intake, providing therapeutic diets, and offering nutrition counselling and support. However, inadequate nutrition personnel, limited nutritional assessment, insufficient therapeutic diet options, poor meal planning, food quality concerns, inadequate monitoring, and weak integration of nutrition services into clinical care may affect the quality of nutritional care provided to hospitalized patients in Nigeria. Against this background, this study investigates the impact of hospital nutrition services on nutritional care among hospitalized patients in Nigeria. The study will be anchored on the Donabedian Model of Healthcare Quality, the Nutrition Care Process, and Systems Theory. The Donabedian Model explains healthcare quality through structure, process, and outcome, with hospital nutrition services representing important structural and process components and nutritional care representing a measurable outcome. The Nutrition Care Process provides a systematic framework involving nutritional assessment, diagnosis, intervention, monitoring, and evaluation to guide individualized nutrition care. Systems Theory emphasizes the interaction among dietitians, doctors, nurses, food-service personnel, patients, hospital management, clinical departments, and food-service systems in determining the quality and continuity of nutritional care. Collectively, these theoretical perspectives provide a suitable framework for explaining how hospital nutrition services may influence nutritional care among hospitalized patients in Nigeria. The study will adopt a quantitative analytical cross-sectional or prospective cohort research design. The study population will comprise hospitalized adult patients receiving care in selected public and private hospitals across Nigeria, as well as healthcare professionals involved in hospital nutrition services. A multistage sampling technique will be used to select states, local government areas, hospitals, wards, clinical departments, healthcare professionals, and eligible hospitalized patients. Hospital nutrition services will be assessed using indicators such as availability of qualified nutritionists or dietitians, nutritional screening at admission, nutritional assessment, individualized meal planning, therapeutic diet provision, meal prescription, dietary counselling, monitoring of food intake, nutrition documentation, multidisciplinary nutrition care, meal delivery systems, food quality, timing of meals, availability of special diets, patient feedback mechanisms, and nutrition follow-up. Nutritional care will be assessed using indicators such as completion of nutritional screening, identification of nutritional risk, adequacy of dietary intake, adherence to prescribed diets, changes in nutritional status, weight monitoring, nutrient intake, management of nutrition-related conditions, documentation of nutritional interventions, and patient satisfaction with nutritional care. Data will be collected using structured questionnaires, nutritional assessment tools, patient medical records, dietary records, meal-service records, nutritionist or dietitian documentation, hospital food-service records, and direct observation where appropriate. Descriptive statistics will be used to summarize patients' demographic and clinical characteristics, nutrition-service availability, nutritional risk, dietary care, and nutritional outcomes. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of hospital nutrition services on nutritional care. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that effective hospital nutrition services have a significant positive impact on nutritional care among hospitalized patients in Nigeria. Patients receiving structured nutrition services are expected to demonstrate better nutritional assessment, improved dietary intake, greater adherence to prescribed therapeutic diets, more appropriate nutrition interventions, and improved monitoring of nutritional status compared with patients receiving limited nutrition support. Early nutritional screening may facilitate identification of patients at risk of malnutrition and enable timely intervention. Collaboration among dietitians, doctors, nurses, pharmacists, and food-service personnel may further improve the coordination and continuity of nutritional care. However, inadequate staffing, limited availability of qualified nutrition professionals, insufficient therapeutic diet options, financial constraints, poor meal quality, inadequate documentation, limited nutrition screening, and weak multidisciplinary collaboration may reduce the effectiveness of hospital nutrition services. The study therefore expects comprehensive, accessible, patient-centred, and adequately integrated hospital nutrition services to contribute significantly to improved nutritional care among hospitalized patients in Nigeria. The study is expected to contribute to the literature on hospital nutrition services, nutritional care, hospital malnutrition, therapeutic diets, nutrition assessment, dietetic services, patient-centred care, healthcare quality, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital management boards, healthcare facilities, dietitians and nutritionists, physicians, nurses, food-service managers, healthcare administrators, professional bodies, development partners, and policymakers regarding strategies for strengthening nutritional care in hospitals. The study will also provide evidence-based recommendations for institutionalizing routine nutritional screening, increasing the availability of qualified nutrition professionals, strengthening individualized therapeutic diet planning, improving hospital food quality and meal delivery, enhancing multidisciplinary nutrition care, strengthening nutrition documentation and monitoring, and integrating nutrition services into hospital quality-improvement programmes across Nigeria.
