Impact of Dementia Screening Outreach on Early Detection of Cognitive Impairment among Older Adults in Nigeria

 

Abstract

Dementia and other forms of cognitive impairment are increasingly important public health concerns among older adults and may affect memory, reasoning, communication, independence, social functioning, and quality of life. Cognitive impairment may remain undetected for extended periods because early symptoms can be subtle, mistaken for normal ageing, or overlooked by family members and healthcare providers. Limited awareness, stigma, inadequate access to geriatric and mental-health services, shortage of trained healthcare professionals, and limited availability of routine cognitive assessment may contribute to delayed identification of cognitive problems in Nigeria. Dementia screening outreach provides an opportunity to take preliminary cognitive assessment closer to older adults within communities and other accessible settings, potentially improving recognition of previously undetected cognitive impairment and facilitating timely professional evaluation. Against this background, this study investigates the impact of dementia screening outreach on early detection of cognitive impairment among older adults in Nigeria. The study will be anchored on the Health Belief Model, Social Ecological Model, and Health Systems Framework. The Health Belief Model explains how older adults' and caregivers' perceptions of susceptibility to cognitive impairment, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in dementia screening. The Social Ecological Model emphasizes the influence of individual, family, community, healthcare, cultural, and environmental factors on access to cognitive health services. The Health Systems Framework emphasizes service delivery, healthcare workforce, health information, medical technologies, financing, accessibility, referral systems, and continuity of care as important components of effective cognitive-health screening programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how dementia screening outreach may influence early detection of cognitive impairment among older adults in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 60 years and above residing in selected urban, semi-urban, and rural communities across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, households, community centres, and eligible older adults. Dementia screening outreach will be assessed using indicators such as frequency of outreach activities, screening coverage, availability of trained personnel, use of standardized cognitive screening tools, screening duration, accessibility of screening locations, participation of family caregivers, health education provided during outreach, referral arrangements, and follow-up procedures. Early detection of cognitive impairment will be assessed using indicators such as abnormal cognitive screening results, suspected cognitive impairment, changes in memory or executive functioning identified through screening, newly identified cases requiring further assessment, referral for comprehensive cognitive evaluation, attendance at follow-up assessments, and clinically confirmed cognitive impairment where appropriate. Data will be collected using structured questionnaires, validated cognitive screening instruments, screening registers, outreach records, referral registers, healthcare facility records, and relevant community health programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, cognitive screening findings, and referral patterns. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the impact of dementia screening outreach on early detection of cognitive impairment. Where a quasi-experimental design is adopted, detection rates before and after implementation of dementia screening outreach may be compared with those of a comparison community or group to determine changes associated with the intervention. Diagnostic performance measures such as sensitivity, specificity, positive predictive value, and negative predictive value may also be assessed where an appropriate clinical reference standard is available. Diagnostic tests will further be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that dementia screening outreach has a significant positive impact on early detection of cognitive impairment among older adults in Nigeria. Older adults who participate in community-based dementia screening outreach are expected to have a greater likelihood of previously undetected cognitive difficulties being identified than those without comparable access to screening. Outreach activities may reduce geographical, financial, and transportation barriers while increasing awareness among older adults and their caregivers about the importance of recognizing cognitive changes. Early identification of abnormal screening results may facilitate timely referral for comprehensive clinical assessment, identification of potentially reversible causes of cognitive symptoms, management of underlying health conditions, supportive interventions, and appropriate planning for future care. Community-based screening may also help reduce misconceptions that memory loss and other significant cognitive changes are always a normal part of ageing. However, stigma, fear of dementia diagnosis, low awareness, limited availability of trained personnel, inadequate screening tools, poor referral and follow-up systems, and limited access to specialist services may reduce effectiveness. The study therefore expects accessible, culturally appropriate, standardized, and professionally supervised dementia screening outreach to contribute significantly to improved early detection of cognitive impairment among older adults in Nigeria. The study is expected to contribute to the literature on dementia screening, cognitive impairment, healthy ageing, geriatric health, community-based screening, mental health, early detection, health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, hospitals, geriatric and psychiatric services, community health workers, older-adult organizations, caregivers, development partners, and policymakers regarding strategies for improving cognitive-health services. The study will also provide evidence-based recommendations for integrating cognitive screening into community and primary healthcare services, training healthcare workers in standardized dementia screening, strengthening referral and follow-up systems, improving public and caregiver awareness of early cognitive symptoms, reducing stigma surrounding dementia, and expanding access to appropriate diagnostic and supportive services for older adults across Nigeria.
