Impact of Bereavement Education on Bereavement Support Knowledge among Healthcare Workers in Nigeria

Abstract

Bereavement is an important emotional, social, and public health concern that may affect individuals and families following the death of a loved one. Healthcare workers frequently interact with bereaved patients and families in hospitals, primary healthcare facilities, maternity units, emergency departments, oncology services, palliative care settings, and other healthcare environments. However, inadequate knowledge of bereavement and appropriate bereavement support may limit healthcare workers' ability to communicate sensitively with bereaved individuals, recognize normal and complicated grief responses, provide appropriate emotional support, and facilitate timely referral when specialized assistance is required. Bereavement education provides an opportunity to strengthen healthcare workers' knowledge of grief, communication with bereaved families, culturally appropriate support, psychological first aid, referral pathways, and professional boundaries. Against this background, this study investigates the impact of bereavement education on bereavement support knowledge among healthcare workers in Nigeria. The study will be anchored on the Dual Process Model of Coping with Bereavement, Social Cognitive Theory, and the Health Belief Model. The Dual Process Model explains how bereaved individuals may move between loss-oriented and restoration-oriented coping processes, providing a framework for understanding different responses to bereavement. Social Cognitive Theory emphasizes the roles of knowledge, self-efficacy, observational learning, communication skills, and professional environment in developing appropriate bereavement support practices. The Health Belief Model explains how healthcare workers' perceptions of the importance and severity of bereavement-related distress, perceived benefits of bereavement education, perceived barriers, and cues to action may influence their acquisition and application of bereavement support knowledge. Collectively, these theoretical perspectives provide a suitable framework for explaining how bereavement education may influence bereavement support knowledge among healthcare workers in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise healthcare workers employed in selected public and private hospitals, primary healthcare centres, maternity facilities, emergency units, palliative care services, and other healthcare institutions across Nigeria. A multistage sampling technique will be used to select states, healthcare institutions, departments, units, and eligible healthcare workers. Bereavement education will be measured using indicators such as exposure to structured bereavement education sessions, training frequency and duration, understanding of grief and bereavement, recognition of normal grief responses, awareness of complicated or prolonged grief, communication with bereaved individuals, emotional support, culturally appropriate care, psychological first aid, end-of-life communication, family support, referral procedures, confidentiality, professional boundaries, self-care, and availability of bereavement support resources. Bereavement support knowledge will be assessed using indicators such as knowledge of grief reactions, recognition of normal and complicated bereavement responses, appropriate communication strategies, active listening, emotional support, culturally sensitive care, psychological first-aid principles, appropriate referral, identification of individuals requiring specialized mental health support, confidentiality, professional boundaries, and knowledge of available bereavement services. Data will be collected using structured questionnaires and appropriately validated bereavement support knowledge instruments administered before and after the educational intervention where a quasi-experimental design is used. Descriptive statistics will be used to summarize healthcare workers' demographic and professional characteristics, previous bereavement education, baseline knowledge, and post-intervention knowledge levels. Inferential statistical techniques, including paired or independent sample tests, chi-square tests, correlation analysis, and multiple regression analysis where appropriate, will be used to determine the impact of bereavement education on bereavement support knowledge. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that bereavement education has a significant positive impact on bereavement support knowledge among healthcare workers in Nigeria. Healthcare workers exposed to structured bereavement education are expected to demonstrate improved knowledge of grief reactions, appropriate communication, emotional support, culturally sensitive care, psychological first aid, referral procedures, and recognition of situations requiring specialized assistance compared with their baseline knowledge or healthcare workers without similar educational exposure. Education may improve healthcare workers' confidence in communicating with bereaved patients and families and help them distinguish expected grief responses from reactions that may require further assessment or professional intervention. Practical case studies, role-playing, communication exercises, and scenario-based learning may strengthen the application of bereavement support principles in clinical settings. Conversely, heavy workloads, emotional demands of healthcare practice, limited training resources, inadequate mental health professionals, cultural differences in bereavement practices, limited referral pathways, and insufficient opportunities for continuing professional development may reduce the effectiveness and sustainability of bereavement education. The study therefore expects structured, practical, culturally sensitive, and sustained bereavement education to contribute significantly to improved bereavement support knowledge among healthcare workers in Nigeria. The study is expected to contribute to the literature on bereavement education, bereavement support knowledge, grief support, healthcare worker training, palliative care, psychosocial support, mental health, patient-centred care, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, state ministries of health, hospital administrators, primary healthcare institutions, palliative care providers, nursing and medical education institutions, healthcare professional bodies, psychologists, psychiatrists, social workers, public health practitioners, development partners, and policymakers regarding strategies for strengthening bereavement support within healthcare services. The study will also provide evidence-based recommendations for integrating bereavement education into healthcare worker education and continuing professional development, strengthening grief-support protocols, improving referral pathways, increasing access to mental health professionals, promoting culturally appropriate bereavement care, and establishing sustainable bereavement support services across healthcare facilities in Nigeria.
