Effect of School Health Referral Education on Healthcare-Seeking Practices among Secondary School Students in Nigeria

Abstract

School health referral systems are an important component of adolescent healthcare because they provide pathways through which students with health concerns can be connected to appropriate healthcare professionals and services. Secondary school students in Nigeria may experience health problems such as infectious diseases, injuries, reproductive and sexual health concerns, mental health challenges, nutritional problems, substance-related issues, and other conditions requiring professional assessment or treatment. However, students may delay seeking care or fail to use appropriate healthcare services because of limited knowledge of referral procedures, misconceptions about healthcare, financial constraints, fear of stigma, concerns about confidentiality, peer influence, reliance on self-medication, and inadequate awareness of available health facilities. School health referral education provides an opportunity to improve students' understanding of when healthcare should be sought, how referrals work, where appropriate services are available, and the importance of timely professional care. Against this background, this study investigates the effect of school health referral education on healthcare-seeking practices among secondary school students in Nigeria. The study will be anchored on the Health Belief Model, Health Literacy Theory, and Social Ecological Model. The Health Belief Model explains how students' perceptions of susceptibility to health problems, perceived severity, perceived benefits of timely healthcare, perceived barriers, self-efficacy, and cues to action may influence healthcare-seeking decisions. Health Literacy Theory emphasizes students' ability to obtain, understand, evaluate, and apply health information when deciding when, where, and how to seek appropriate healthcare. The Social Ecological Model highlights the influence of individual, family, peer, school, community, healthcare, socioeconomic, and environmental factors on students' healthcare-seeking practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how school health referral education may influence healthcare-seeking practices among secondary school students in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise students aged approximately 12–19 years enrolled in selected public and private secondary schools across Nigeria, with the specific age range determined according to the approved study protocol and ethical requirements. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, communities, schools, classes, and eligible students. School health referral education will be assessed using indicators such as exposure to referral education sessions, frequency and duration of education, knowledge of common health problems requiring professional assessment, recognition of health warning signs, understanding of school referral procedures, knowledge of appropriate healthcare facilities, awareness of school health personnel, referral documentation, emergency referral pathways, confidentiality considerations, healthcare-seeking options, and available adolescent-friendly health services. Healthcare-seeking practices will be assessed using indicators such as timely consultation with qualified healthcare professionals, use of recommended healthcare facilities, compliance with school referrals, attendance at referred appointments, appropriate use of primary healthcare services, seeking professional care for persistent or severe symptoms, avoidance of unnecessary self-medication, appropriate emergency healthcare-seeking, follow-up after referral, and use of reliable health information when making healthcare decisions. Data will be collected using structured questionnaires, standardized healthcare-seeking and referral knowledge assessment tools, age-appropriate scenario-based questions, school health and referral records where ethically and practically accessible, and pre-test and post-test assessments where a quasi-experimental intervention is adopted. Descriptive statistics will be used to summarize students' demographic and educational characteristics, common health concerns, sources of health information, referral awareness, and healthcare-seeking practices. Inferential statistical techniques, including chi-square tests, paired and independent t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of school health referral education on healthcare-seeking practices. Where a quasi-experimental design is adopted, healthcare-seeking practice scores before and after the educational intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that school health referral education has a significant positive effect on healthcare-seeking practices among secondary school students in Nigeria. Students exposed to structured, age-appropriate, practical, and accessible referral education are expected to demonstrate greater use of appropriate healthcare services than students without comparable exposure. Education may improve students' ability to recognize symptoms requiring professional assessment, understand referral procedures, identify appropriate healthcare facilities, and seek care promptly when necessary. It may also reduce inappropriate reliance on self-medication, informal advice, or delayed healthcare-seeking. Scenario-based education may strengthen students' ability to determine appropriate actions in situations involving persistent illness, injuries, reproductive and sexual health concerns, mental health difficulties, or other health problems. However, financial constraints, transportation difficulties, parental consent requirements, stigma, concerns about confidentiality, peer influence, limited availability of adolescent-friendly services, inadequate school health personnel, and long distances to healthcare facilities may reduce the effectiveness of education alone. The study therefore expects accessible, practical, sustained, and school-integrated health referral education, supported by functional referral pathways and adolescent-friendly healthcare services, to contribute significantly to improved healthcare-seeking practices among secondary school students in Nigeria. The study is expected to contribute to the literature on school health services, healthcare-seeking behaviour, referral systems, adolescent health, health education, health literacy, primary healthcare utilization, adolescent-friendly healthcare, preventive health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Education, National Primary Health Care Development Agency, state ministries of health and education, secondary schools, school health personnel, healthcare professionals, primary healthcare centres, community health workers, adolescent-health organizations, parents, development partners, and policymakers regarding strategies for strengthening school-based health referral systems. The study will also provide evidence-based recommendations for integrating referral education into school health programmes, strengthening school-to-health-facility referral pathways, improving students' knowledge of appropriate healthcare services, reducing barriers to adolescent healthcare utilization, strengthening communication between schools and healthcare facilities, improving confidentiality and referral follow-up, and developing sustainable school health interventions that promote timely and appropriate healthcare-seeking among secondary school students across Nigeria.
