Effect of Newborn Eye Screening on Early Detection of Eye Disorders among Newborns in Nigeria
Abstract
Newborn eye disorders remain an important but often under-detected public health concern because some congenital and early-onset eye conditions may not produce obvious symptoms during the first weeks of life. Conditions such as congenital cataract, congenital glaucoma, retinopathy of prematurity, ocular infections, structural abnormalities, and other visual disorders can affect normal visual development and may result in avoidable childhood visual impairment or blindness when detection and treatment are delayed. Early identification of eye disorders during the newborn period provides an opportunity for timely referral, diagnosis, treatment, and follow-up before permanent visual damage occurs. Newborn eye screening provides a systematic approach to identifying potential ocular abnormalities through appropriate examination and screening procedures carried out shortly after birth or during the early neonatal period. However, newborn eye screening coverage remains limited in many Nigerian settings because of inadequate awareness, shortages of trained personnel, limited screening equipment, weak referral systems, financial constraints, and poor integration of newborn eye screening into routine maternal and newborn healthcare services. Against this background, this study investigates the effect of newborn eye screening on early detection of eye disorders among newborns in Nigeria. The study will be anchored on the Health Systems Framework, Screening Theory, and Health Belief Model. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and governance as essential components of effective healthcare delivery. Screening Theory provides a framework for identifying diseases or health conditions at an early stage among individuals who may not yet show obvious clinical symptoms, thereby supporting timely intervention and improved health outcomes. The Health Belief Model explains how parents' or caregivers' perceptions of the susceptibility and severity of newborn eye disorders, perceived benefits of screening, perceived barriers, and cues to action may influence acceptance and utilization of newborn eye screening services. Collectively, these theoretical perspectives provide a suitable framework for explaining how newborn eye screening may influence the early detection of eye disorders among newborns in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise newborns delivered in selected hospitals, maternity centres, and primary healthcare facilities across Nigeria. A multistage sampling technique will be used to select states, local government areas, healthcare facilities, newborns, and their mothers or caregivers. Newborn eye screening will be measured using indicators such as availability of screening services, screening coverage, timing of screening, screening frequency, availability of trained healthcare personnel, availability of appropriate screening equipment, screening procedures used, and referral arrangements. Early detection of eye disorders will be assessed using indicators such as number of newborns screened, number of abnormal eye findings identified, confirmed eye disorders, age at detection, referral for specialist assessment, and timely initiation of appropriate management. Data will be collected using structured questionnaires, newborn screening records, hospital registers, ophthalmic examination records, referral records, and relevant programme documents. Descriptive statistics will be used to summarize newborn characteristics, screening coverage, screening outcomes, and patterns of eye disorder detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of newborn eye screening on early detection of eye disorders. Where appropriate, detection rates before and after implementation of newborn eye screening programmes may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that newborn eye screening has a significant positive effect on the early detection of eye disorders among newborns in Nigeria. Newborns who receive appropriate eye screening are expected to have a higher likelihood of having congenital or early-onset eye abnormalities identified promptly than newborns who do not receive screening. Early screening may facilitate the identification of conditions such as congenital cataract, congenital glaucoma, ocular infections, structural abnormalities, and other disorders that may otherwise remain undetected until visual impairment becomes apparent. Timely identification may also improve referral to ophthalmologists and other relevant specialists and allow appropriate treatment or monitoring to begin at an early stage. Conversely, limited screening coverage, inadequate equipment, shortages of trained healthcare personnel, delayed referrals, poor parental awareness, and weak integration of eye screening into routine newborn services may reduce the effectiveness of screening programmes. The study therefore expects systematic newborn eye screening to contribute significantly to early detection and timely management of eye disorders among newborns in Nigeria. The study is expected to contribute to the literature on newborn eye screening, neonatal health, childhood eye health, congenital eye disorders, early disease detection, ophthalmic public health, maternal and child health, and preventive healthcare in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, teaching hospitals, primary healthcare centres, ophthalmologists, paediatricians, midwives, nurses, maternal and newborn health programme managers, development partners, and policymakers regarding strategies for strengthening newborn eye health services. The study will also provide evidence-based recommendations for integrating newborn eye screening into routine neonatal care, expanding screening coverage, training healthcare workers in basic newborn eye assessment, improving access to appropriate screening equipment, strengthening referral pathways, increasing parental awareness, improving specialist eye-care availability, and establishing effective follow-up systems to support early diagnosis and management of newborn eye disorders across Nigeria.
