Effect of Medicine Adherence Education on Medication-Taking Practices among Adults with Hypertension in Nigeria

Abstract

Hypertension is a major public health concern in Nigeria and an important risk factor for stroke, heart disease, kidney disease, and premature mortality. Although effective antihypertensive medicines are available, failure to take prescribed medicines correctly and consistently may reduce treatment effectiveness and contribute to poor blood pressure control and preventable complications. Medication non-adherence among adults with hypertension may be associated with inadequate knowledge, forgetfulness, concerns about adverse effects, misconceptions about long-term medicine use, treatment costs, complex medication regimens, perceived absence of symptoms, and limited communication between patients and healthcare providers. Medicine adherence education provides an opportunity to improve patients' understanding of their prescribed medicines, correct misconceptions, address barriers to adherence, and encourage appropriate medication-taking practices. Against this background, this study investigates the effect of medicine adherence education on medication-taking practices among adults with hypertension in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Health Literacy Framework. The Health Belief Model explains how adults' perceptions of hypertension severity, perceived benefits of medication adherence, perceived barriers, susceptibility to complications, and cues to action may influence their medication-taking behaviour. Social Cognitive Theory emphasizes self-efficacy, behavioural reinforcement, observational learning, communication, and practical skills in developing and maintaining appropriate medication-taking practices. The Health Literacy Framework emphasizes patients' ability to access, understand, evaluate, and apply information about prescribed medicines when making decisions concerning their treatment. Collectively, these theoretical perspectives provide a suitable framework for explaining how medicine adherence education may influence medication-taking practices among adults with hypertension in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise adults aged 18 years and above who have been diagnosed with hypertension and are receiving antihypertensive treatment in selected hospitals, primary healthcare centres, and community-based healthcare settings across Nigeria. A multistage sampling technique will be used to select geopolitical zones, states, local government areas, healthcare facilities, clinics, and eligible participants. Medicine adherence education will be assessed using indicators such as frequency and duration of education sessions, explanation of prescribed medicines, dosage and timing instructions, importance of consistent medication use, management of common medication-related concerns, information on possible adverse effects, strategies for remembering doses, medicine storage, refill planning, communication with healthcare providers, and follow-up education. Medication-taking practices will be assessed using indicators such as adherence to prescribed dosage and timing, consistency of daily medication use, missed doses, timely medicine refills, appropriate medicine storage, use of reminders, avoidance of unauthorized dose changes or discontinuation, communication with healthcare providers about adverse effects, and attendance at scheduled treatment follow-up visits. Where appropriate, medication adherence may also be assessed using validated adherence scales, pharmacy refill records, pill counts, or other objective measures available within the study setting. Data will be collected using structured questionnaires, standardized medication-adherence assessment tools, patient treatment records, pharmacy refill records, and relevant healthcare facility documents. Descriptive statistics will be used to summarize participants' characteristics, hypertension history, exposure to adherence education, and medication-taking practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of medicine adherence education on medication-taking practices. Where a quasi-experimental design is adopted, medication-adherence scores before and after the educational intervention may be compared with those of a comparison group to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that medicine adherence education has a significant positive effect on medication-taking practices among adults with hypertension in Nigeria. Adults exposed to structured adherence education are expected to demonstrate better compliance with prescribed medication regimens than adults without comparable exposure. Education may improve patients' understanding of why antihypertensive medicines must often be taken consistently even when they feel well, and may help them understand the importance of following prescribed dosage and timing instructions. Practical education on medication reminders, refill planning, medicine storage, and communication with healthcare providers may further improve adherence. Addressing misconceptions and concerns about adverse effects may also reduce inappropriate discontinuation or alteration of prescribed treatment. However, medication costs, medicine availability, forgetfulness, complex treatment regimens, adverse effects, inadequate patient–provider communication, and limited access to follow-up care may continue to affect adherence despite education. The study therefore expects accessible, patient-centred, culturally appropriate, and continuous medicine adherence education to contribute significantly to improved medication-taking practices among adults with hypertension in Nigeria. The study is expected to contribute to the literature on medication adherence, hypertension management, patient education, health literacy, chronic disease management, pharmaceutical care, primary healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, Pharmacy Council of Nigeria, state ministries of health, hospitals, primary healthcare centres, physicians, pharmacists, nurses, community health workers, public health practitioners, development partners, and policymakers regarding strategies for improving hypertension treatment adherence. The study will also provide evidence-based recommendations for integrating medicine adherence education into routine hypertension care, strengthening patient counselling services, developing simple and culturally appropriate medication education materials, improving refill and follow-up systems, training healthcare workers in adherence counselling, and addressing practical barriers to consistent antihypertensive medication use across Nigeria.
