Effect of Laboratory Quality Management Systems on Diagnostic Accuracy in Nigerian Medical Laboratories
Abstract
Accurate laboratory diagnosis is essential for effective healthcare delivery because laboratory test results guide disease diagnosis, treatment decisions, patient monitoring, disease surveillance, and public health interventions. In Nigerian medical laboratories, diagnostic accuracy may be affected by inadequate equipment, poor quality-control procedures, insufficiently trained personnel, specimen-handling errors, reagent shortages, equipment malfunction, poor documentation, and inconsistent adherence to standard operating procedures. Laboratory Quality Management Systems provide a structured approach to managing laboratory processes through quality assurance, quality control, standard operating procedures, staff competency assessment, equipment maintenance, document control, internal audits, and continuous quality improvement. Effective implementation of these systems may reduce errors across the pre-analytical, analytical, and post-analytical phases and improve the accuracy and reliability of laboratory results. Against this background, this study investigates the effect of Laboratory Quality Management Systems on diagnostic accuracy in Nigerian medical laboratories. The study will be anchored on the Donabedian Model of Healthcare Quality, Total Quality Management Theory, and Systems Theory. The Donabedian Model explains healthcare quality through structure, process, and outcome, with laboratory infrastructure and quality management systems representing structural components and diagnostic accuracy representing a measurable outcome. Total Quality Management Theory emphasizes continuous improvement, standardization, staff involvement, quality control, customer focus, and organizational commitment to quality. Systems Theory views the laboratory as an interconnected system in which personnel, equipment, specimens, procedures, information, and organizational processes interact to influence the quality of diagnostic results. Collectively, these theoretical perspectives provide a suitable framework for explaining how Laboratory Quality Management Systems may influence diagnostic accuracy in Nigerian medical laboratories. The study will adopt a quantitative analytical cross-sectional or comparative laboratory-based research design. The study population will comprise selected public and private medical laboratories and laboratory professionals involved in diagnostic testing across Nigeria. A multistage sampling technique will be used to select states, local government areas, medical laboratories, laboratory departments, diagnostic sections, laboratory scientists, technicians, and relevant test records. Laboratory Quality Management Systems will be assessed using indicators such as availability and implementation of standard operating procedures, internal quality control, external quality assessment participation, equipment calibration and maintenance, reagent quality management, staff competency assessment, document and record control, internal audits, corrective and preventive actions, biosafety procedures, specimen management, quality indicators, and continuous quality-improvement activities. Diagnostic accuracy will be assessed using indicators such as agreement between laboratory results and reference or confirmatory methods, sensitivity, specificity, accuracy rate, false-positive results, false-negative results, error rates, specimen rejection rates, and concordance with established diagnostic standards where applicable. Data will be collected using structured questionnaires, laboratory assessment checklists, quality-management records, internal audit reports, external quality-assessment results, equipment maintenance records, quality-control logs, standard operating procedures, and relevant laboratory test records. Descriptive statistics will be used to summarize laboratory characteristics, quality-management practices, staffing, equipment availability, and diagnostic performance. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of Laboratory Quality Management Systems on diagnostic accuracy. Diagnostic performance measures will also be calculated for relevant laboratory tests, while appropriate reliability and validity assessments will be conducted to ensure robustness of the findings. The study is expected to find that effective implementation of Laboratory Quality Management Systems has a significant positive effect on diagnostic accuracy in Nigerian medical laboratories. Laboratories with stronger quality management practices are expected to demonstrate lower diagnostic error rates, improved concordance with reference methods, fewer false-positive and false-negative results, and more reliable laboratory results than laboratories with weaker quality systems. Standardized procedures, regular internal quality control, external quality assessment, equipment maintenance, staff competency assessment, appropriate specimen management, and systematic corrective actions may reduce errors throughout the laboratory testing process. However, inadequate funding, shortages of trained laboratory personnel, irregular supply of reagents, poor equipment maintenance, unreliable electricity, limited access to external quality-assessment programmes, and weak institutional support may reduce the effectiveness of laboratory quality management. The study therefore expects comprehensive, consistently implemented, and adequately supported Laboratory Quality Management Systems to contribute significantly to improved diagnostic accuracy in Nigerian medical laboratories. The study is expected to contribute to the literature on Laboratory Quality Management Systems, diagnostic accuracy, laboratory quality assurance, laboratory quality control, medical laboratory services, diagnostic errors, patient safety, healthcare quality, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Medical Laboratory Science Council of Nigeria, state ministries of health, hospital management boards, medical laboratories, laboratory scientists, healthcare providers, accreditation bodies, development partners, and policymakers regarding strategies for improving laboratory diagnostic quality. The study will also provide evidence-based recommendations for strengthening Laboratory Quality Management Systems, expanding external quality-assessment participation, improving equipment maintenance and calibration, ensuring consistent availability of quality reagents, strengthening staff competency assessment, improving internal audits, implementing corrective and preventive actions, and promoting continuous quality improvement across Nigerian medical laboratories.
