Effect of Community Sanitation Campaigns on Open Defecation among Rural Households in Nigeria

Abstract

Open defecation remains an important environmental and public health challenge in Nigeria, particularly in rural communities where access to improved sanitation facilities may be limited. The practice of defecating in open spaces can contribute to environmental contamination and increase the risk of exposure to fecal pathogens, diarrhoeal diseases, intestinal infections, and other adverse health outcomes. Rural households may continue to practice open defecation because of inadequate toilet facilities, poverty, cultural practices, limited sanitation infrastructure, low awareness of associated health risks, and poor maintenance of available facilities. Community sanitation campaigns provide an opportunity to promote improved sanitation behaviour through health education, community mobilization, hygiene promotion, advocacy, household visits, sanitation demonstrations, local leadership involvement, and promotion of household latrine construction and use. Against this background, this study investigates the effect of community sanitation campaigns on open defecation among rural households in Nigeria. The study will be anchored on the Social Ecological Model, Health Belief Model, and Community Organization Theory. The Social Ecological Model emphasizes the interaction between individual, household, community, environmental, and policy factors in shaping sanitation practices. The Health Belief Model explains how household members' perceptions of susceptibility to sanitation-related diseases, perceived severity, perceived benefits of using toilets, perceived barriers, and cues to action may influence sanitation behaviour. Community Organization Theory emphasizes community participation, collective decision-making, local leadership, empowerment, and mobilization in addressing community health problems. Collectively, these theoretical perspectives provide a suitable framework for explaining how community sanitation campaigns may influence open defecation practices among rural households in Nigeria. The study will adopt a quantitative quasi-experimental or community-based comparative research design. The study population will comprise households residing in selected rural communities in Nigeria. A multistage sampling technique will be used to select states, local government areas, rural communities, households, and eligible household respondents. Community sanitation campaigns will be assessed using indicators such as frequency of sanitation education sessions, household visits, community meetings, hygiene demonstrations, distribution of sanitation education materials, community mobilization activities, involvement of traditional and community leaders, promotion of household latrine construction, sanitation monitoring, community participation, and exposure to sanitation messages through local media. Open defecation will be assessed using indicators such as household practice of open defecation, frequency of open defecation, availability and use of household toilets, access to functional sanitation facilities, observable evidence of open defecation around households and communities, toilet-sharing practices, and household sanitation behaviour. Data will be collected using structured questionnaires, household observation checklists, community sanitation records, environmental health assessment forms, and programme monitoring documents. Where feasible, household sanitation facilities will be physically observed to supplement self-reported information. Descriptive statistics will be used to summarize household characteristics, sanitation facilities, exposure to community campaigns, and open-defecation practices. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of community sanitation campaigns on open defecation. Where a quasi-experimental design is adopted, open-defecation practices before and after implementation of sanitation campaigns may be compared, or households in intervention communities may be compared with households in similar communities without enhanced sanitation campaigns. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community sanitation campaigns have a significant negative effect on open defecation among rural households in Nigeria, indicating that effective sanitation campaigns may reduce the practice. Households exposed to regular sanitation education, community mobilization, household visits, and toilet-use promotion are expected to demonstrate greater use of available sanitation facilities and lower levels of open defecation than households with limited exposure to sanitation campaigns. Community participation and involvement of traditional and local leaders may strengthen acceptance of sanitation messages and encourage collective action. However, inadequate toilet infrastructure, poverty, lack of water supply, cultural beliefs, poor maintenance of sanitation facilities, overcrowding, weak enforcement of sanitation regulations, and limited programme resources may reduce the effectiveness of community sanitation campaigns. The study therefore expects sustained, culturally appropriate, participatory, and adequately supported community sanitation campaigns, combined with improved access to functional sanitation facilities, to contribute significantly to reducing open defecation among rural households in Nigeria. The study is expected to contribute to the literature on community sanitation campaigns, open defecation, rural sanitation, water sanitation and hygiene, environmental health, hygiene promotion, community health education, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Water Resources and Sanitation, Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health and environment, local government authorities, environmental health officers, community health workers, traditional leaders, development partners, non-governmental organizations, and policymakers regarding strategies for improving sanitation practices. The study will also provide evidence-based recommendations for strengthening community sanitation campaigns, expanding household access to affordable and functional toilets, increasing community participation, improving sanitation education, strengthening household visits and monitoring, engaging traditional and community leaders, addressing cultural and socioeconomic barriers, and integrating sanitation campaigns with broader rural water, sanitation and hygiene programmes to reduce open defecation across Nigeria.
