Effect of Community-Based Hearing Aid Assessment on Hearing Rehabilitation Needs Detection among Older Adults in Nigeria
Abstract
Hearing loss is an important health and social concern among older adults in Nigeria and may affect communication, social participation, emotional well-being, independence, and quality of life. Age-related hearing loss may develop gradually, causing affected individuals and their families to adapt to reduced hearing without recognizing the extent of the problem or the need for rehabilitation. Limited awareness, financial constraints, geographical barriers, stigma associated with hearing devices, and inadequate access to audiological services may further contribute to unmet hearing rehabilitation needs. Community-based hearing aid assessment provides an opportunity to identify older adults who may require hearing rehabilitation and further audiological evaluation through primary healthcare centres, community outreach programmes, elderly support groups, home-based care services, and other accessible community platforms. Early identification of hearing rehabilitation needs may facilitate comprehensive hearing assessment, hearing-aid counselling, appropriate device selection, fitting, follow-up, and referral to audiologists or other qualified hearing-care professionals. Against this background, this study investigates the effect of community-based hearing aid assessment on hearing rehabilitation needs detection among older adults in Nigeria. The study will be anchored on the Health Belief Model, International Classification of Functioning, Disability and Health (ICF) framework, and Social Ecological Model. The Health Belief Model explains how older adults' perceptions of hearing loss, perceived severity of communication difficulties, perceived benefits of hearing rehabilitation, perceived barriers, and cues to action may influence participation in hearing assessment and acceptance of rehabilitation services. The ICF framework recognizes the interaction between health conditions, body functions, activity limitations, participation restrictions, and environmental factors in determining the functional consequences of hearing loss and the need for rehabilitation. The Social Ecological Model emphasizes the influence of individual, family, community, socioeconomic, cultural, and healthcare-system factors on hearing health and access to rehabilitation services. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based hearing aid assessment may influence the detection of hearing rehabilitation needs among older adults in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise adults aged 60 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, primary healthcare centres, elderly support groups, and eligible older adults. Community-based hearing aid assessment will be measured using indicators such as availability of assessment services, assessment coverage, frequency of assessment activities, hearing difficulty assessment, communication difficulty assessment, functional hearing assessment, use of appropriate hearing screening procedures, availability of trained hearing-care personnel, hearing-aid needs assessment, counselling, device-use education, affordability assessment, and referral arrangements. Hearing rehabilitation needs detection will be assessed using indicators such as identification of older adults with hearing difficulties requiring further evaluation, communication-related functional limitations, suspected need for hearing aids, newly identified hearing rehabilitation needs, previous hearing-aid use, need for hearing-aid fitting or replacement, need for audiological assessment, referrals to audiologists or hearing-care professionals, and documented rehabilitation recommendations following appropriate assessment. Data will be collected using structured questionnaires, validated hearing assessment instruments, hearing screening procedures, communication and functional assessment tools, community health records, hearing rehabilitation registers, and referral documentation. Descriptive statistics will be used to summarize participants' demographic characteristics, hearing difficulties, communication limitations, assessment coverage, and patterns of rehabilitation needs detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of community-based hearing aid assessment on hearing rehabilitation needs detection. Where appropriate, rehabilitation needs detection rates before and after implementation of community-based hearing assessment activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based hearing aid assessment has a significant positive effect on hearing rehabilitation needs detection among older adults in Nigeria. Older adults who participate in systematic community-based hearing assessment are expected to have a higher likelihood of previously unrecognized hearing rehabilitation needs being identified than older adults without access to such services. Routine assessment may facilitate early recognition of hearing difficulties, communication limitations, reduced social participation, and functional problems that may indicate the need for hearing rehabilitation. Early detection may provide opportunities for comprehensive audiological evaluation, hearing-aid counselling, appropriate device selection, fitting and adjustment, communication strategies, family education, and follow-up care. Bringing hearing rehabilitation assessment closer to older adults may reduce geographical, financial, transportation, and access barriers and may reach individuals who rarely attend specialized audiology services. Community-based hearing-health education may also reduce stigma surrounding hearing aids and improve awareness of available rehabilitation options. Conversely, high device costs, limited availability of trained audiologists, inadequate hearing-assessment equipment, stigma, low awareness, financial constraints, and weak referral and follow-up systems may reduce the effectiveness of community-based hearing aid assessment. The study therefore expects accessible, systematic, and appropriately implemented community-based hearing aid assessment to contribute significantly to improved detection of hearing rehabilitation needs among older adults in Nigeria. The study is expected to contribute to the literature on community-based hearing aid assessment, hearing rehabilitation needs detection, older adults' hearing health, age-related hearing loss, audiological rehabilitation, communication health, healthy ageing, community health services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, audiologists, otolaryngologists, hearing-care professionals, nurses, community health workers, older adults' organizations, development partners, and policymakers regarding strategies for improving access to hearing rehabilitation. The study will also provide evidence-based recommendations for expanding community-based hearing assessment, strengthening referral pathways to audiological services, increasing availability of affordable hearing rehabilitation, improving hearing-aid counselling and follow-up, reducing stigma associated with hearing devices, strengthening community hearing-health education, and integrating hearing rehabilitation needs assessment into appropriate community-based healthy-ageing and primary healthcare programmes across Nigeria.
