Effect of Community-Based Breast Health Screening on Breast Abnormality Detection among Women in Nigeria

Abstract

Breast abnormalities remain an important public health concern among women in Nigeria and may include breast lumps, nipple abnormalities, breast pain, skin changes, and other changes that may require further clinical evaluation. Some breast abnormalities may be associated with benign conditions, while others may indicate breast cancer or other serious breast diseases. Delayed identification of suspicious breast changes can result in late diagnosis, delayed treatment, increased healthcare costs, and poorer health outcomes. Women may experience barriers to early breast assessment because of limited awareness, fear of diagnosis, financial constraints, geographical barriers, cultural beliefs, stigma, and inadequate access to appropriate healthcare services. Community-based breast health screening provides an opportunity to bring breast health assessment closer to women through community outreach programmes, primary healthcare centres, mobile health services, women's health campaigns, and other community platforms. Early detection of breast abnormalities may facilitate timely clinical breast examination, diagnostic imaging, biopsy where indicated, referral, treatment, and continued follow-up. However, inadequate screening coverage, shortages of trained healthcare personnel, limited diagnostic equipment, low awareness, financial barriers, and weak referral systems may limit the effectiveness of community-based breast health screening programmes in Nigeria. Against this background, this study investigates the effect of community-based breast health screening on breast abnormality detection among women in Nigeria. The study will be anchored on the Health Belief Model, Community Health Theory, and the Health Systems Framework. The Health Belief Model explains how women's perceptions of susceptibility to breast abnormalities, perceived severity, perceived benefits of screening, perceived barriers, and cues to action may influence participation in breast health screening. Community Health Theory emphasizes the delivery of accessible health services through community structures and the involvement of communities in disease prevention, health promotion, and early detection activities. The Health Systems Framework emphasizes service delivery, health workforce, health information, medical products and technologies, financing, and accessibility as essential components of effective breast health screening and healthcare programmes. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based breast health screening may influence breast abnormality detection among women in Nigeria. The study will adopt a quantitative cross-sectional analytical or quasi-experimental research design. The study population will comprise women aged 18 years and above residing in selected communities across Nigeria. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible women. Community-based breast health screening will be measured using indicators such as availability of screening services, screening coverage, frequency of community screening activities, accessibility of screening locations, number of women screened, breast health education, clinical breast examination, availability of trained healthcare personnel, availability of screening equipment, and referral arrangements. Breast abnormality detection will be assessed using indicators such as detection of breast lumps, breast masses, nipple abnormalities, skin changes, breast pain, abnormal clinical breast examination findings, suspected breast abnormalities requiring further assessment, referrals for diagnostic imaging or specialist examination, and confirmed breast conditions following appropriate clinical evaluation. Data will be collected using structured questionnaires, community screening registers, clinical breast examination records, diagnostic reports where available, referral registers, healthcare facility records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, screening coverage, breast examination findings, and patterns of breast abnormality detection. Inferential statistical techniques, including chi-square tests, correlation analysis, and logistic or multiple regression analysis, will be used to determine the effect of community-based breast health screening on breast abnormality detection. Where appropriate, detection rates before and after implementation of community-based screening activities may be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based breast health screening has a significant positive effect on breast abnormality detection among women in Nigeria. Women who participate in community-based breast health screening are expected to have a higher likelihood of having previously unrecognized breast abnormalities identified than women without access to such services. Community-based screening may facilitate early identification of breast lumps, masses, nipple abnormalities, skin changes, and other suspicious findings and provide opportunities for timely clinical assessment, diagnostic imaging, specialist referral, and appropriate treatment where necessary. Bringing breast health screening services closer to women may reduce geographical, financial, and time-related barriers and increase awareness of the importance of seeking care for unusual breast changes. Early detection may improve the likelihood of timely management of serious breast conditions and may reduce complications associated with delayed diagnosis. Conversely, inadequate screening coverage, limited diagnostic facilities, shortage of trained healthcare personnel, low awareness, fear of diagnosis, cultural barriers, financial constraints, and weak referral and follow-up systems may reduce the effectiveness of community-based breast health screening. The study therefore expects accessible and well-organized community-based breast health screening to contribute significantly to improved detection and early management of breast abnormalities among women in Nigeria. The study is expected to contribute to the literature on community-based breast health screening, breast abnormality detection, women's health, breast cancer prevention, early detection, community health services, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Primary Health Care Development Agency, state ministries of health, primary healthcare centres, hospitals, breast health and cancer-care providers, community health workers, women's health organizations, development partners, and policymakers regarding strategies for improving early detection of breast abnormalities. The study will also provide evidence-based recommendations for expanding community-based breast health screening programmes, improving access to clinical breast examination and appropriate diagnostic services, strengthening community breast health education, increasing availability of trained healthcare personnel and screening resources, improving referral and follow-up systems, reducing financial and geographical barriers, addressing fear and stigma surrounding breast abnormalities, and promoting early presentation for breast health assessment among women across Nigeria.
