Effect of Community-Based Blood Donation Campaigns on Voluntary Blood Donation among Adults in Nigeria

Abstract

Adequate availability of safe blood remains essential for effective healthcare delivery and the management of conditions such as severe anaemia, obstetric haemorrhage, traumatic injuries, surgical emergencies, and other life-threatening conditions. Nigeria continues to face challenges in maintaining sufficient supplies of safe blood because of low rates of voluntary blood donation, dependence on family replacement and paid or directed donations in some settings, misconceptions about blood donation, fear of adverse effects, inadequate awareness, and limited access to convenient donation opportunities. Community-based blood donation campaigns provide an important public health strategy for mobilizing eligible adults, increasing awareness of the importance of voluntary blood donation, addressing misconceptions, and bringing blood donation opportunities closer to communities. However, inadequate campaign coverage, limited community mobilization, logistical challenges, insufficient donor education, and weak follow-up systems may affect the effectiveness of these campaigns. Against this background, this study investigates the effect of community-based blood donation campaigns on voluntary blood donation among adults in Nigeria. The study will be anchored on the Health Belief Model, Theory of Planned Behavior, and Social Ecological Model. The Health Belief Model explains how adults' perceptions of the need for blood, perceived benefits of donation, perceived barriers, perceived susceptibility to health emergencies, and cues to action may influence their decision to donate blood voluntarily. The Theory of Planned Behavior explains how attitudes toward blood donation, subjective norms, perceived behavioural control, and intention may influence voluntary blood donation behaviour. The Social Ecological Model emphasizes the influence of individual, family, community, organizational, cultural, socioeconomic, and environmental factors on blood donation practices. Collectively, these theoretical perspectives provide a suitable framework for explaining how community-based blood donation campaigns may influence voluntary blood donation among adults in Nigeria. The study will adopt a quantitative analytical cross-sectional or quasi-experimental research design. The study population will comprise eligible adults aged 18 years and above residing in selected Nigerian communities. A multistage sampling technique will be used to select states, local government areas, communities, households, and eligible participants. Community-based blood donation campaigns will be assessed using indicators such as campaign frequency, campaign coverage, duration, health education activities, community mobilization, use of media and interpersonal communication, availability of convenient donation locations, involvement of community leaders, donor counselling, eligibility information, and follow-up activities. Voluntary blood donation will be assessed using indicators such as participation in voluntary blood donation, first-time donation, repeat donation, number of donations within a specified period, willingness to donate voluntarily in the future, donation without direct replacement or personal request, and registration as a voluntary blood donor where applicable. Data will be collected using structured questionnaires, blood donation campaign records, blood bank registers, donor registration records, and relevant programme documents. Descriptive statistics will be used to summarize participants' characteristics, campaign exposure, knowledge and attitudes toward blood donation, and donation patterns. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of community-based blood donation campaigns on voluntary blood donation. Where a quasi-experimental design is adopted, voluntary blood donation rates before and after implementation of community-based campaigns may be compared, while relevant demographic, socioeconomic, and community factors are appropriately controlled. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that community-based blood donation campaigns have a significant positive effect on voluntary blood donation among adults in Nigeria. Adults exposed to regular and well-organized community-based campaigns are expected to demonstrate greater awareness of the importance of blood donation and a higher likelihood of voluntarily donating blood than adults with limited exposure to such campaigns. Community campaigns may address misconceptions and fears, improve knowledge of donor eligibility and safety, increase perceived benefits of donation, and make donation opportunities more accessible. Involvement of community leaders, healthcare workers, religious and social organizations, and local communication channels may further strengthen community participation. However, fear of needles, concerns about weakness after donation, misconceptions about blood donation, eligibility restrictions, inconvenient donation locations, inadequate donor incentives, poor campaign organization, and limited follow-up may reduce participation. The study therefore expects sustained, accessible, culturally appropriate, and community-driven blood donation campaigns to contribute significantly to increasing voluntary blood donation in Nigeria. The study is expected to contribute to the literature on community-based blood donation campaigns, voluntary blood donation, blood donor recruitment, blood safety, community mobilization, health education, healthcare services, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, National Blood Service Agency, state ministries of health, blood transfusion services, healthcare facilities, community health workers, community leaders, voluntary organizations, development partners, and policymakers regarding strategies for improving voluntary blood donation. The study will also provide evidence-based recommendations for expanding community-based donor recruitment campaigns, strengthening blood donation education, improving donor counselling, increasing the accessibility and convenience of donation sites, strengthening repeat-donor follow-up systems, engaging community leaders and organizations, addressing misconceptions and fears, and integrating voluntary blood donation promotion into broader public health and blood safety programmes across Nigeria.
