Effect of Cervical Cancer Screening Education on Screening Knowledge among Female Civil Servants in Nigeria

Abstract

Cervical cancer remains an important public health concern among women and is largely preventable through vaccination, regular screening, early detection, and appropriate management of precancerous lesions. Despite the availability of cervical cancer screening methods such as human papillomavirus (HPV) testing, visual inspection with acetic acid (VIA), and cytology-based approaches, many women may have limited knowledge about cervical cancer risk factors, screening recommendations, available screening methods, and the importance of early detection. Female civil servants represent an important population for health promotion because they are accessible through organized workplaces and may benefit from workplace-based health education and screening initiatives. However, misconceptions, fear of screening, perceived lack of symptoms, stigma, limited awareness, cost, and concerns about access to screening services may contribute to low screening knowledge and uptake. Cervical cancer screening education provides an opportunity to improve women's understanding of cervical cancer prevention and encourage appropriate utilization of screening services. Against this background, this study investigates the effect of cervical cancer screening education on screening knowledge among female civil servants in Nigeria. The study will be anchored on the Health Belief Model, Social Cognitive Theory, and Theory of Planned Behavior. The Health Belief Model explains how female civil servants' perceptions of susceptibility to cervical cancer, perceived severity of the disease, perceived benefits of screening, perceived barriers, and cues to action may influence their acquisition of screening knowledge and subsequent screening decisions. Social Cognitive Theory emphasizes learning through education, observation, social interaction, self-efficacy, and environmental influences in the development of health-related knowledge and behaviours. The Theory of Planned Behavior explains how knowledge, attitudes, subjective norms, and perceived behavioural control may influence women's intention to utilize cervical cancer screening services. Collectively, these theoretical perspectives provide a suitable framework for explaining how cervical cancer screening education may influence screening knowledge among female civil servants in Nigeria. The study will adopt a quantitative quasi-experimental or analytical cross-sectional research design. The study population will comprise female civil servants working in selected federal, state, and local government ministries, departments, and agencies across Nigeria. A multistage sampling technique will be used to select states, government establishments, departments, and eligible female civil servants. Cervical cancer screening education will be assessed using indicators such as exposure to structured educational sessions, frequency and duration of education, information on HPV infection, cervical cancer risk factors, signs and symptoms, prevention, available screening methods, recommended screening intervals, eligibility for screening, screening procedures, interpretation of screening results, and appropriate follow-up. Screening knowledge will be assessed using participants' understanding of cervical cancer causes and risk factors, the relationship between HPV and cervical cancer, the purpose of screening, available screening methods, recommended screening practices, the importance of screening in asymptomatic women, possible screening results, and the need for appropriate follow-up after abnormal findings. Data will be collected using structured questionnaires and validated cervical cancer screening knowledge assessment instruments. Descriptive statistics will be used to summarize participants' characteristics, exposure to screening education, and levels of screening knowledge. Inferential statistical techniques, including chi-square tests, t-tests, correlation analysis, and logistic or multiple regression analysis where appropriate, will be used to determine the effect of cervical cancer screening education on screening knowledge. Where a quasi-experimental design is adopted, screening knowledge scores before and after the educational intervention will be compared to determine changes associated with the intervention. Diagnostic tests will also be conducted to assess the reliability, validity, and robustness of the findings. The study is expected to find that cervical cancer screening education has a significant positive effect on screening knowledge among female civil servants in Nigeria. Female civil servants exposed to structured and evidence-based cervical cancer screening education are expected to demonstrate higher levels of knowledge regarding HPV infection, cervical cancer risk factors, prevention, screening methods, eligibility, screening procedures, and the importance of early detection than those with limited exposure to screening information. Education may correct misconceptions that cervical cancer screening is only necessary when symptoms are present and may improve awareness of the preventive role of screening in identifying precancerous changes before invasive cancer develops. Improved knowledge may also increase confidence in discussing screening with healthcare providers and encourage informed decisions regarding screening. However, fear of screening procedures, misconceptions about cervical cancer, stigma, limited access to screening facilities, cost, competing work responsibilities, and concerns about privacy may reduce the effectiveness of educational interventions. The study therefore expects accessible, evidence-based, workplace-appropriate, and regularly reinforced cervical cancer screening education to contribute significantly to improved screening knowledge among female civil servants in Nigeria. The study is expected to contribute to the literature on cervical cancer screening education, screening knowledge, women's health, HPV prevention, cancer prevention, workplace health promotion, preventive healthcare, and public health in Nigeria. The findings will provide useful information to the Federal Ministry of Health and Social Welfare, Federal Ministry of Women Affairs and Social Development, Federal Ministry of Labour and Employment, state ministries, government ministries and agencies, workplace health programmes, healthcare providers, cancer-control organizations, development partners, and policymakers regarding strategies for improving cervical cancer prevention among female civil servants. The study will also provide evidence-based recommendations for integrating cervical cancer screening education into workplace health programmes, strengthening employee health education, improving access to affordable screening services, training healthcare educators, developing culturally appropriate educational materials, increasing awareness of HPV and cervical cancer prevention, and promoting appropriate screening and follow-up among female civil servants across Nigeria.
